Accented, all of them. Since most speech in the story is in English and it's not their native tongue, they tend to speak in a slow/deliberate tone that's sometimes not entirely intelligible. If you take Geert for instance, the Dutch accent is particularly noticeable by the way she pronounces her 'r's and 'g's. As for Dilip, him being relatively high-society makes it less noticeable, though still noticeable when he gets flustered. It's still a farcry from actual Engrish.

Hopefully in that case the initial release would be minimal, only to let out the artificial pressure before it evens out with the depth pressure at the outlet. That being said, while many wells were old, there still are some new wells that are being surveyed/pumped nowadays. These would still have a lot of pressure in them regardless of injectors.
That being said, it's those newer wells that are more likely to be fitted with a turreted connection. Hence the decreased rupture/sudden disconnection risk. Deepwater Horizon only occured relatively recently (2010), and that was on a new well.
However, that safety is only temporary. When the materials wear out enough because of rust, a catastrophic rupture can be expected.

RAS stands for 'Replenishment At Sea', it's the proper NATO name for Underway Replenishment (I think that would be the term the USN uses). It's tricky to execute for all teams involved. On my side (navigation), we have to monitor the approach yard per yard (FYI, all of NATO uses yards for nav manoeuvres, even metric countries like mine) and make sure we're sailing clear of any vessels because the operation doesn't make RAS ships 'stand on' vessels according to COLREG.
On the deckworks side, shooting the tether (via rifle grenade) can sometimes go wrong real quick. Let it be said that radomes are not impervious to grappling hooks.
