I would use minor threats first. Like maybe applying some kind of chemical that causes intense irritation and itching to a part of the body first, preferably somewhere delicate, and then move onto worse stuff each time she doesn't back off.
7273588 Eh... Poison joke is, unless the magic it's filtering is particularly awful, nonlethal and overall harmless. Kinda like how the poison joke on the border of the Everfree caused humorous but not dangerous effects, whereas the field from the climax of the last story actually killed a filly because there was just that much bad mojo.
So frankly unless Tarnish drops the effigy in a field in the Scarriest Cave in Equestria I think she'd be fine, if really /really/ hating him.

I'm starting to think they should call Twilight in to help, or at the very least get some high powered antibiotics. I suppose zebra magic could suffice? And would be a doubly effective opportunity to showcase Tarnish's development and intertwine him and Starlight.
Also, glad to finally see some sympathetic magic! No one in this setting seems to touch on rote magic at all (except maybe the Apples?), but everyone's all about intent, and it's nice to see something fresh.

I hope you realise that, as I've just finished a Emergency First Aid course, I was shouting "Why are you being so moronic? GET HELP, YOU IDIOT!" at the screen. Then he pulled out that nail, which is a HUGE no-no. I was thinking he deserved the faceful of pus, but that could have been plugging an arterial punture.

They need to take Trixie to a hospital because it's clear that she's been sick for days and has a lot of injuries on top of that. I suspect that she might also be malnourished which wouldn't help her recovery at all. Oh and none of them are actually doctors so they're not prepared for this and one mistake could make things worse.

Well, so long as we're being gross... I confess to like popping my zits. It's like bubble wrap. There's just something strangely satisfying the sound, the feel of the rupture, and sensation of the unwanted substance being forcibly evicted from my being. This might be connected to my OCD, or just me being me. In any case, I have absolutely no qualms popping my own sacks of puss. However, I once watched a video of a zit the size of my big toe being cut open in a hospital and it nearly made me gag, so I probably shouldn't be asked to pop anyone else's.
