It is a premise of confusion for one filly/mare. As a recolor of a character, it is telling the mental thoughts of Sweetie Belle, living as BonBon, and vice versa. She doesn't know who she truly is

I have to agree it almost sounds like a short Highlander type fic “there can only be one” but hard to tell with just one chapter.
From simple inspiration come the strangest sorts of stories. Gripping bit of existential tragedy. Thank you for a wonderfully weird little tale and an enjoyably ominous ending.
