What I gathered was that he passed out because of a physical injury like a concussion.
However, him being exhausted makes more sense, due to the fact that he can survive breaking the sound barrier.

For one the spell requires whatever species Twilight wants to turn into is present when she cast the spell, In other words, Accell has to be present for the spell to work. And second, the spell is not on a timer, the spell last as Twilight decides to break the spell.
