Very interesting start here, and then the whole thing flows smoothly. I know it's a one shot and all, but the way it's written gives it the potential for more. Very nice.
I agree with Keradon this is a prime example of a one shot that shows much potential for expansion. Please consider it.
Interesting take on the whole "Where is Diamond Tiara's mom" situation. Most fics have her dead, either through childbirth, some kind of illness, or an accident. Having her as a mental patient is pretty unique.

This is the second story I see with DT's mom as Screw Loose. Nicely done!
All aboard the feels train! Toot toot! Aside from a few random typos I really don't have anything to criticize c:
A lovely story, well written, no glaring errors. I would actually like to see more from this.
Also, I can think of a few ways this could actually have a happy ending.
For one thing, show canon does show us that magic can do a lot beyond just telekinesis, which makes it sort of the obvious choice, though if it were simple the unicorn doctors would probably have fixed it. So, there's a few sources of mind-kerwaggity we've seen in the show that could be worked toward:
1) Changelings - Queen Chrysalis and changelings at large seem capable of (with sufficient exposure and feeding) mind-affecting magic. Presumably they would be skilled in this venture, and could concievably piece back together a broken mind, at least to a degree.
2) Luna - Dreams are a window into the mind and the subconscious. It's possible through dream-walking the condition could be treated, or even cured. Hay, grab Diamond Tiara, Silver Spoon, and Applebloom and go along with her.
3) Discord - For a reality warper capable of causing any amount of insanity, it stands to reason that with proper incentive he could repair damage as well. How you'd convince him to do so is beyond me. Presumably by going through Fluttershy.
4) Zecora - If you want to stick to the bittersweet, consider a potion to offer temporary return of sanity. If you wanted to make it a tearjerker, you could even have it only work once. or work with less effect each time. This could then segue into one of the other options, or end there.

Or, you could leave her the way she is, so she doesn't recover. It would be a bit of a happy-sad ending if DT gets some form of closure via AB, but still doesn't have her mom.
