Its wings, she just grew wings. The blue patch on her back was the clue. Also a horn would mean blood down her face and she brought it up to her eyes. So... Wings... Or maybe... wings and horn?

Tearing and splattering. Skin doesn't make a tearing sound. Clothes do. I'm guessing a pair of wings, at the very least.
Edit (to this part) - Skin does make some small amounts of sound, but nothing that simple activities like talking or walking wouldn't muffle.

From the skin tearing and having the wings themselves, or at least the bones, sliding out from one's back, i imagine there would be at least some blood. The same could be said of a horn, though i doubt that she'd not see the blood running down her face and trying to get into her eyes.

Actual real-life science. If you could somehow use chemical polarity or electric-based docking to connect each individual nano-nerve fibre (implying that our biotech level or nanotech level can create such sophisticated and effective products, which we won't have for a few decades if Scientific American and other good scientific inquiries are anything to go by) to the actual biological nerve network, how would the body react to it? I really hope that by the time I've been through college and in the work force for 5-10 years that we'll at least have some semblance of research in that area (of course, it's obviously not that simple-- if it was, we would have something like that by now) that I can marvel over.
Just think about it: would we have to "program" a prosthetic to interact with the nerves, or would your body instantly "install" the necessary information and act as if the machinery (or regrown organic part) was completely natural? Oh, what I'd give to talk to experts in this field just to see if my ideas are mere science fiction currently, or if they're actually viable thoughts into such an advanced science...

That whole scenario was just great, though no one has mentioned one of my favorite parts yet; how Luna bribed the guards, "extra cider, extra pay, and not having to serve a rotation in Ponyville." Because as we all know, down in Ponyville, CRAZY things happen on a regular basis.
Am I the only one that had a relatively mundane reaction to the end of that? I kinda expect it to be Celestia hitting Elam with a sonic blast in return for that embarrassment, rupturing Elam's tympanic membranes or some such.

Wouldn't the brain slowly adapt to the new appendage over time, though (like learning a new task, only your brain would slowly come to figure out how the new appendage functions and moves)? I seem to recall the brain being very versatile and adaptable, although the context it was in was in (where if you say, cut your hand off and they re-attached it fast enough, it could eventually heal and become usable again) was a bit different than what I'm thinking of.
That's the other thing though. If you think about the nervous system as a closed circuit (which is kinda is), adding a new electrical pathway would just have the nerve impulses be directed through it as well, while still allowing it function at the same effectiveness. Granted, the biochemistry behind it is obviously more advanced than just simple electrical physics concepts, but you get the idea.
Overall, my thoughts are just that-- thoughts. Unfortunately, I'm no PhD or expert in any of these fields (not yet at least ) and most of my knowledge, despite being decent, is barely even the tip of the iceberg. Therefore, I'm probably missing at least one small piece of the puzzle... heck, I may be missing a section or even half the puzzle.
