cured the minds body probelm but now she has a distrust of others due to celestia.
Well... I expected a lot worse from the title.
Still churns my guts, wondering what's going to happen next.

This for me, as well.
Accepting his fate as a mare. Making the best of whats happened to him. I understand it, but I still hate it. At least he still has that fear about the magic in his mind. It did remove the dysmorphia, but is that all it did? How much did Twilight, much less Celestia, know about it? Are there other effects? No wonder he is afraid he no longer has any choices.
Nice bait and switch. I liked this chapter, felt like a breath of fresh air.
One of my cousins did that.
Ooh, i sense somepony in need of some aloe vera.
Now them's words to live by.
Not expecting a time skip
OK, Now for my actual opinion on the story itself. While it starts out rough, It cleans up very well as the chapters progress and becomes very engaging. I love the themes and I enjoy how the truth and plot unfolds without being obvious on whats going on. This really is well done.
That being said, the first couple of chapters are a mess. This guy acts like a lunatic and is all over the place with little interaction and no real reasons. Ill be honest, its like the Author said, "OK, these things have to be in the story, so lets just get them the hell out of the way so we can get on with it." So the trite concepts of "Oh my god I'm in a new world, Oh my god, i'll never get home, oh my god your a horse, oh my god im loosing my crap because reasons. are just dumped hap hazard into the chapter. And make no mistake they are slapped in. Very few of his responses and actions make any sort of sense. Especially in responses to what actually happens in the chapters. For example, he freaks out about not being able to go home before anyone even raises the issue at all.
While this story does suffer from a staggery and bizarre start, it does get its act together and pulls off a very engaging and thought provoking tale.
Well done to the Author and keep up the good work, Im very much looking forward to more.

“Puberty was a curse for those inflicted with it, and boundless amusement for others who survived the process.” -Scarheart
Toffee is right, there is no choice. She doesn't actually feel comfortable, she doesn't love her body, nothing about her is real. She decided to stay a mare because Celestia, the one who brought Gerry to Equestria murdered him by destroying his identity and forcing him to choose and want what Toffee wants, which is only what Celstia wants.
And nobody ever doing anything avoit it means both Twilight and Celestia can never redeem themselves. What's even worse is that Applejack never doing anything means that in the end even she has fallen from grace and abandoned friendship. At this point, the only solution is for Toffee to die.

1. Twilight already did all she could, and so did Applejack(which wasn’t much since she can’t do magic nor does she have enough of an emotional connection to Celestia to be able to have an impact).
2. I’m pretty sure she still has free will for the most part, all Celestia did was remove her dysphoria and even out her temper a bit, not replace her mind with a set of pre-programmed responses
3. How would toffee dying solve anything?
No matter what you do,
No matter how you try,
Someone's gonna lose,
Someone's gonna die.
