I'd love a sequel where the cake twins and Twilight's friends find a way to reverse the spell but can't perform it cause Velvet was enjoying being a "Mother" again and she begs for more time to be in that role again and they grant her one more day to be the Mother she missed being to Twily and Shining.
If you want to go a little dark though have it that Nightmare Moon comes back to get revenge on Twilight sees her regressed and sees how much fun Velvet is having but she can also sense that Velvet is sad because she knows her children can't be foals forever and she will feel useless again so the Nightmare possess Velvet while she is sleeping and starts corrupting Velvet's maternal instinct. While at the same time poisoning Velvet's mind with twisted answers to her own questions like "i feel useless to my children" to which Nightmare responds with "but i don't have too now they need me now" to which Velvet says "but they are adults not foals" to which Nightmare "no they will always be foals if i run away with them and never let anypony find us then i can be the mother i enjoy being" and other corrupting words to eventually break Velvet into doing the act of running away and hiding from the world to raise her foals and keep them foals. Then end the story with a final scene where Velvet's eyes flash the cyan blue slitted eyes of Nightmare Moon before returning to her original purple telling the reader that as of right now the Nightmare has fully taken control of Velvet's mental facilities and will keep influencing Velvet
