Here some ideas of you can do to Spike the "teenage" baby dragon:
1. Spike get a full physical body examination and I talking about a full hoof-on examination like them casually touching his huge Dragon junk and checking his butthole and maybe even using a rectal thermometer.
2. Spike is forced to sit in a small baby seat while he is forced to eat bady food.
3. Twilight give Spike a bath in a small baby tub while she give him a thorough washing.
4. Spike is forced to sleep in a small baby crib or bed.
5. Twilight take Spike out shopping with her with Spike being forced to sit in a small baby stroller.
6. Ember show up in Ponyville because Twilight ask Ember to babysit Spike within the Dragonlands for a few days while Twilight is busy aiding the Princesses with something. Ember treat Spike as a baby dragon as she take him back to the Dragonlands and while Spike is trying to talk to her but Ember mistake as being Spike hungry so she force-feed him in the way how Dragons feed their young by taking food and regurgitated it up and forcing it into Spike's mouth.
6.1. While in the Dragonlands Ember left with Garble and his pals to watch Spike while she goes off to do some important Dragonlord stuff. And surprisingly treated Spike well (Garble may be a huge jerk but he not going be a total dick to a baby Dragon even though he kinda finds them annoying sometimes) Garble and his pals treat kinda like a baby sibling and Garble is constantly giving Spike noogies. Garble and his pals take Spike out with the boys to show him how a real Dragon is like and while Garble have Spike sit on his shoulders with Spike's huge junk are pressing the back of Garble's head and while Garble and his pals play some "grown-up" Dragon games they left Spike to with the other baby Dragons. And at some point Garble try using Spike to get some hot Dragon babes by showing them how good he is with babies. And multiple times they and Ember punish Spike by having him lay on their lap and giving him a hand to the rear spanking.
7. Spike is always punished by getting a naked bottom spanking with Spike laying on their lap.
8. Rarity have Spike wear small cute baby outfits (most of the outfits are for young girls) that she have made for him and they don't nothing in covering his private parts due to how small the outfit are completely exposing them for all to see as Spike is forced to wear them out in public.

Let me go down the list.
1: Maybe not, as I’m not good with doctor scenes, though that is an interesting idea.
2, 3, 4, and 5: Was already planning those, except they’d be just big enough to fit our “baby” dragon, but I like your thinking.
6 and 6.1: Was actually thinking of a way to incorporate the dragons, thank you.
7: Don’t worry, pretty much every spank Spike gets will be on his bare scaly butt.
8. Sounds interesting enough to try. Problem is I forgot to add that Spike wears clothes, so I’ll fix that.
Edit: Fixed it, so now it’ll be more embarrassing when he gets those.

Here some more suggestions:
1. Rarity forced Spike to wear small cute outfits for little girls something like
here and here except the skirts are very short for his size and they don't go down to his hips which expose his crotch and rear for all to see.
2. And while Spike is being babysit by Ember and the other dragons in the Dragonlands Spike does not wear any diaper due to baby Dragons in the Dragonlands do not wear diapers. But that also means that Spike be completely naked while his junk is hanging out completely in the open which will leads to some uncomfortable and suggestive situations like Garble having Spike ride on his shoulders his huge Dragon junk bounces and being press in the back of Garble's head.
3. Twilight take Spike to the School of Friendship and they bump into the Students Six. While Twilight busy at the school she have the Students Six watch Spike while she works so they show Spike around the school to show how important his "mommy's" works is for everycreature at the school then they take him to the school's grounds so they can play and while playing some inappropriate and suggestive shenanigans happen due to Spike really being a teenage Dragon but they're unable to see that.
4. Mrs. Cup Cake force-feed Spike her breast milk directly from her breast like a baby when she run out of milk to feed Spike with.
That all of my suggestions for now.

1. twilight get spell/pill so she produce milk in breast can breastfeed him.
2. be fun if few pony won't effect by discord magic say like rarity or kids so they still make joke at him wear diaper as teenager.
3. take spike to crystal empire to have playdate with furry heart and he still seen as hero by a diaper dragon hero baby. while twilight get distracted by guard. spike could expire a few up and down while wear the diaper.
like use the diaper as way carry gems in the diaper as bag.
4. spike could wake a differ size, or twilight could take vist it her parents.

3: Don’t really have a good understanding about them and their personalities aside from Gallus and Smolder. And not really interested in trying to write down Yona’s speech pattern. (Also meaning, as much as I love her, Zecora will not be appearing in anything I write anytime soon) But I’m willing to give it a go for that scenario, could be fun.

1: I am interested fully in making Spike breastfeed, I’m just not entirely sure how to write those scenes.
2: I’m actually making a thing that will explain what the spell Discord cast actually does in detail, that will be among what’s talked about.
3: Yeah, I’ll give the “Baby Hero” a break and let him still have saved the Crystal Empire, just the circumstances will be much different than what happened, and he won’t have been the only one to do so. And that could be very fun.
4: Spike will be the same size the entire time, but I will consider a visit to her parents.
All of that sounds awesome~ maybe have discord throw in some involuntary bowel movements? I thought you might make it that the dragons from the Dragonlands would always walk around in the complete nude because they have no concept of wearing clothes or modesty and male dragons have huge human cock and balls with no sheaths which leaves their privates fully expose something like in these pics here Derpibooru: #1454918 and #555316. And because Spike was raised by ponies he have modesty thus making him feel very awkward due to him being completely naked around other naked dragons and very uncomfortable when he is surrounded by a bunch of naked dragon dudes thats always have their junk out on display.
If you decide to have Spike get breastfed you can give the mares crotch-boobs.
I don't know who should I feel more sorry for, Spike for everyone seeing and treating as a baby or King Sombra who think that he have been defeated by a baby dragon that no older then a foal.
Can you make it that Spike not wear a diaper while he is wearing small baby clothes that are made by Rarity and other cute outfits? So that way Spike is even more embarrassed (that make wish he was wearing a diaper) by having him wear small girly outfits that leaves his privates expose for all to see (of course they aren't aware of his expose junk they think are seeing a baby dragon wearing small baby clothes that fit him. But the thought of his junk being expose for everyone to see make him feel embarrass and anyone that can see him as a teenager, like the CMCs will make fun of him for wearing small girly clothes and having his junk out on display) while out in public.

That dragon one is good, but I was going with the idea that they’d be wearing something, and Spike would be the only one naked. But now, I’m getting the idea of having Spike sitting on their lap.
That was the plan for the breastfeeding.

Okay, thanks for the heads up. And I too also really like the idea of Spike sitting many laps of a bunch of naked teenage dragons while their huge junk are uncomfortably poking him in the rear or his back. I can see this happening to Spike a lot of times while being in the Dragonlands and even see it happen while Garble and boys are using Spike to pick up some chicks.
Maybe if you wanna go the pleasure route you could have spike get erections is his diapers from the padding so twilight and others could rub the front of his diaper till he cums and just have it rationalized in their mind that he just tired himself out after getting so excited from the attention. However everypony would still snicker and giggle at the sight of the drake being rubbed to climax. Love the meeting idea~ could also throw in some straight jacket bondage in case he was being fussy and struggling ealier in the day and after the tantrum gets something to fill his mouth to muffle him. I’m just spitballing so feel free to ignore this if it is outside your comfort zone.
The POV is from Spike right? So he can't know things like that. He can only deduce it from their reactions. It would be better I think if you moved this line to after Spike notices the foals laughing. Then instead of a universal POV it is his own thought.
If you shift perspective from Spike to a universal one you need a new paragraph. If you don't want to do that you can keep it in Spike's perspective such as "He just could barely contain his frustration....". These things would be going through his mind as with his strength just him putting force on a child's toy would be enough to break it.
I don't know if Twilight would really do something like that so easily. It feels a little forced. Maybe you could make it feel more natural by having Spike snap at her. After all he just accidentally broke the toy in anger so he is plenty agitated. That would make Twilight's reaction feel more natural and justified.
It's a little awkward with Spike's behavior being mentioned retroactively. You would do better mentioning it before the conversation so that the reader will imagine him thrashing while they read it.

That seems kind of unnecessary. The adults see Spike as a baby they aren't going to believe the kids. After all if you see a baby and a 8 year old tells you it's an adult do you believe him?
One fun rule could also be that spankings are considered the appropriate punishment for Spike and they are always seen as justified no matter how administers them.

though try to make the reactions of different characters diverse. It helps greatly to prevent the story getting too repetitive. So I would remove rule that all the children laugh at him. That they are all themselves. However children are by nature mischievous and sometimes a little cruel. If they know they can bully someone without consequence they often are tempted to try. Sometimes even those who are otherwise nice.
For example if Sweetie Belle just had a fight with Rarity and she stumbles across Spike. Spike says a poorly worded word of comfort and that makes Sweetie take her frustrations out on him.

I do hope you will try. I always like seeing the different reaction of the characters. AB would have a very tough love attitude towards it and likely be Spike´s main ally. However she would enjoy being in charge of him and even punish him. SB will more so treat him as a plaything, putting him in all kinds of outfits and acting out scene´s. Scootaloo would be the one who would bully him most as she would try to employ the same tactics as RD but doesn't completely understand them.
I don't exactly understand how that would work. If the clothes are revealing because they don't fit it would be very uncomfortable for Spike. Maybe instead make it that Rarity only makes clothes for him that leave his everything below his midsection exposed. For easy changes and the like.
Also I kind of want her to make him something like a onesie so maybe instead you could make it that Rarity makes only foal clothes or those that keep his diaper available.
