yes, I know it is confusing and to be honest, that was the whole point. this is a fever dream after all :p
So based on what the story intales im gonna take a crack at deciphering it so based on all the events it seems that 5 years ago on nightmare night applebloom wanted to make carmel apples with the cauldron but messed up set fire to the barn killing her and her two friends after which it spread into sweet apple acres killing all of the apple family and finally spreading and most likely killing all of if not most of ponyville and the ending with twilight would be spike simply dreaming since she's already been dead and gone for so long and the reason spike survived is do to the fact that he is a dragon and it seems appleblooms ghost is haunted by that one mistake since she knows that it was her fault and obviously the filly in the story was her and she killed everyone with one simple accident
