I've only read this chapter, I'll admit, but I have to confess while I'm curious about the premise, I'm a bit turned off over the whole "everyone is in on the secret society" thing.
If its so common, why is it a secret at all? If anything, it seems implied that by and large its a socially accepted system (BDSM, that is). Indeed, the last part seems to imply the whole country was founded on those ideals on some level.
I suppose on some level I'm just objecting to the idea of completely rewriting everyone and everything in order to put forth a premise like this. It's cliche.

If even just 1/10th of the populace were in on it, it would be part of mainstream society after ~1,000 years of practice.
To put that in perspective, there are only ~10 million people playing Pokemon Go (1 in 700, or about 0.14%) in the world, so imagine it were nearly 80x as popular...
And based on how many of them Twilight knows personally just in her little town, 10% is an impossibly low estimate.
One in ten is the same odds of a person being left handed.
(And to put that in perspective, how many lefties do you know or meet in every day life? A LOT less than the number of ponies Twilight is recognizing from town, I would wager.)
If I had to guess, I'd say anywhere between 35% to 75% of the population are in on it, if not more.
(The mane six, a group of totally random mares that only have embodiment of Elements in common, show that the percentage is roughly 66%, if you choose to use them as a random metric. Granted, this isn't even close to a good metric to use, but it's the most accurate one we have after just this prologue chapter.)

As counterpoint--we don't know if the situation is similar outside of Ponyville. More realistically, with two clock towers you see two communities grow up around them. The majority of Equestria doesn't have a clock tower and as far as we know may not a single member of it anywhere else but those two places.
As to the thousands of years would make it mainstream argument... We don't know that for sure. We have an ancient statue hinting at ancient practices. We have ancient sources depicting all kinds of rituals and magic we don't do today... Except for those who try to, and they aren't mainstream even if the ideas are old and never quite vanished. In between Founding and now, we have no idea how the history of these rites/rituals/societal arrangements fared. For that matter, so far I'm not sure which it is--is this a depiction of a romantic-sexual bond like a sort of political marriage? Was this an arrangement ritualized in anyway?
I'm curious so far.

A secret sex society can be viewed as a metaphor of the real-life sex industry. A large portion of society takes great pains to shelter innocent little children from learning about the carnal horrors of sexual intercourse, which encourages the sex industry to remain hidden from casual glances. Many of those innocent little children grow into innocent little adults (like Twilight), unaware of the extent of sexual culture unless someone else introduces them, or they get curious. You don't generally encounter things like sex toy shops or porn stores unless you're actively looking for them; they're hidden in plain sight.
A fictional story depicting something like the Clocktower Society just makes this open-secret aspect of sex more explicit.

Alright then. Heard enough about this and read a few related stories. Now that I'm finally seeing it, I'm intrigued. I especially like how you're working in the herds, as well as the kinda-but-not-really global secret of the Clocktower.
"By the Fires of Devotion, your Word is Law." sounds like something an edgy anime protagonist or a tokusatsu hero would say.

I'm not crazy about Doctor Horse, and I'm finding the "everyone's in on it but Twilight" thing hard to swallow. Like... How? Why? As a person who is more than happy to ship multiple of the main 6 in sexy shenanigans, I'm still cognizant of the fact that (if we're aiming for characterization reasonably not too far from the show) not ALL of them would go for kink let alone the entire town. It just flies too much in the face of canon. I don't think it would be too much to say this deserves an alt. universe tag what with the massive presence and power this 'clocktower society' has, and how OOC the characters have to be to participate in it...
I'm going to keep reading in the hopes that there is enough of a logical justification for all of this, or else I may have to quit this before I see much kink happening.

So this is my first time looking at a Clocktower fic. I've read all the comments on this chapter and it seems people have picked up on the obvious flaw in the premise - there is simply no way all this could be hidden. It's not even remotely imaginable, my dude. That said, I think your prose is actually pretty decent and given the fact you started a mere one month on this site before I did and this was done in 2016 I can imagine you've developed your tradecraft substantially since producing this fair effort.
This actually reminds me of one of my canceled stories, Foolers Rush In which started with a neat concept and then jackknifed about 30,000 words in because I foolishly changed direction midstream to write another idea I had instead. In that story, there was a bar where fillyfoolers went to get laid and everypony in town actually did know about it so there didn't exist this problem of plausibility (even Roseluck, the town bachelorette/nymphomaniac knew about the place). It's not until the story has been going awhile where disbelief slowly creeps in by author intent paying off with the big reveal the characters are in a dream. The story goes on to reference World of Warcraft and there's dream demon bat pony alicorn and oh god did that story go to hell in a handbasket. Fuck me!
I feel like if I'd have combined that story concept with this one it would actually have been really freaking good! The bar isn't a dream but a physical location funded by a pony with a lot of pull and money sorta like how Gentleman For Mares played it. Play down the number of locals that are present in the sex party club, built in-house transports so they don't have to parade through town like a horny mob to get to the place and voila!
I'm going to keep going just to see how the sex goes but this is definitely giving me ideas! As I said, the prose isn't too bad. The story could use editing but it's still fairly readable. The suspension of disbelief issues exist but would be easy as hell to retcon.
Honestly if I was investigating this issue in the same manner as Twilight my suspicions would go out the window when someone mentioned a guest list and gave me a pass.
