I like the concept of Cadance's responsibilities, and like to believe that medicine in general is Cadance's domain really, since she's the only one ever shown to actually cast something that could be called a healing spell (her bubble in Three's a Crowd).
I feel sorry for her, really, i hope this turns out to be a story about finally finding peace and moving on rather than continued depravity.
Maybe once she sees other ponies for their own worth rather than relative to the one she's assigned to the line she's fixed upon she'll be free, painful as that might be.
It's mostly just me and some elderly archivists, and nothing very exciting usually happens there. But it does give me a lot of control over Equestria's genealogical scholarship, which has allowed me to cover my tracks and ensure that my special project isn't discovered. I place little smudges at strategic locations, and make sure that only the most proficiently unreadable of academics are entrusted with studying certain lineages, to ensure proper commitment to rigor.
It's also allowed me to effectively search all of Equestria for suitable prospects for my darlings.

5819711 It... actually makes sense. In the comic canon Candance has charts and figures that explain exactly why they are compatible. An immortal being cares not for things like age difference. After all, no matter how old he gets, he cannot compare to her true age regardless. So she would bide her time and wait until she could place herself in his path.
With Shining, she did have to work more than with others, what with the changelings and all. I imagine she was quite pissed at Crysalis for that.
Actually, the changelings strike me as being antithetical to everything Candance is. They steal true love and replace it with lies.
5819689 Actually, I think what will do it will be realizing that - with Shining dead, her having no foals by him, and Twilight an alicorn - that the line is basically ended. Velvet and Night Light - despite her previous thoughts - are unlikely to attempt to have another foal, especially because of the emotional effect it would have on Twilight (trying to fill the void by 'replacing' Shining, Twilight would throw a fit over that). Twilight is unlikely to pursue foals of her own for several decades, if not longer, given she's likely to bury herself in research to hold the pain at bay. With the line all but ended, the plan is over...and all that's left is moving on, and she'll have to, if only to prevent Twilight from falling into the same mental trap she did.
A slow spiral into... well, not quite madness. Obsession, certainly, but Cadance is at least putting up a facade of sanity good enough to fool Celestia. Still, she isn't exactly the picture of mental health. And there's no telling what might happen later on, should a certain purple princess reject her...
