He will likely need less protection for his legs, reducing weight of worn apparel. His ears swivel, likely increasing his hearing. In short, joke's on the weed. Ian won.

There is a reason we have feet. They work well for our bipedal gait. His tailbone emerging into a tail instead of acting like a cradle for his guts means that he has lost some internal robustness. Humans can take stupid amounts of injuries and bounce back, compared to many other animals that just... die. On the other hand, the tail seems functional. It's also attracting a mate? Yay tail!
Hearing will be... different. Horse ears are good at hearing specific things, and human ears are good at hearing other specific things. Pony ears, on the other hoof... a bit of both. They are also talkers, so they will need to be attuned to speech, so at least he has that going on. If they were actual horse-horse ears, speech would become quieter and harder to hear in the fidelity a human would want for communication.

I may be wrong about this, but I was under the impression that it's the sacrum that does most of the work, with the coccyx being almost entirely vestigial and providing little to no benefit. I don't think he's lost anything (aside from his pride, perhaps) by gaining a tail.

Oh-ho. Well, I'm certain that'll upset the paradigm a bit. Celestia will be due for a little bit of a freakout once Ian gets back. Thankfully, he has pants on still, so she won't get a show at the same time!
If Ian continues falling for Water Lily though, once he returns to Canterlot he may have to see if some more Poison Joke won't complete the trick. It probably doesn't work like that, but one never knows.
