A have a friend who writes. He just sits down and thinks, and if he can't come up with something, he just does something else for a few hours. Seems to work pretty well for him.

Instead of everyone living, everyone is already dead and is in some kind of afterlife. Some kind of events of your choosing happen and people begin being revived. Instead of people trying not to die, the story is about people clinging to death to avoid life.
