I've had a theory since the first few chapters, and some information presented in this chapter supports this theory. Like Mesmer, I have no idea if this theory is true, but it would make a lot of sense if it were. It certainly makes me more excited to find out if I'm right!

So the mastermind has likely driven Fluttershy insane and made her kill herself, Discord and an entire orphanage (or maybe made her watch Pinkie's death)... and Pinkie has been missing for a while now. Rainbow is in deep denial. I wonder what the other three have been doing.

Your theory may indeed be correct. A couple tidbits relating to it have been mentioned in earlier chapters... But that particular reveal is far off. Still, keep it in mind.

Mesmer is a tough cookie with an iron will and perhaps the greatest mental stability of any of the characters. Make of that what you will.
Finally caught up on this, and what a wild ride it's been. I really have no idea what to make of Inky's revelation, much as I've been poring over it. I do have a theory though, that
is talking about suicide, and that all game participants have somehow been tagged as high risk for attempting it, so it's saying 'if you try to off yourself (heaven) I'll put you in the killign game (hell). Maybe his logic is "if they're going to end their lives anyways, then I'm not shortening their lifespan with my game, and ergo, haven't actually done anything wrong." And this makes me strongly believe that Patience is Dr. _______ from the article. Just seems like a mastermind thing to do, put yourself in (semi) plain sight to taunt the participants. His being a psychologist would explain how he had access to the records he'd need to keep finding participants. As for why he's doing this, maybe it's somehow to the ends of finding a cure for depression - killing some wolves to save the flock.
I saw an online post discussing murder as a form of sending children to heaven and it immediately made me think of this. This is my favorite chapter, so I just had to read it again. I even caught a detail I missed the first few times, which could affect that old theory of mine.
