I'd recommend it. You don't technically need to play it, but a bit of shit makes sense when it comes to some stuff that's talked about, like mechanoids, psykers, insectoids, etc.. I've personally dumped nearly four hundred hours in the game, the first hundred and fifty without mods. It's pretty good for folks that like colony sims with a bit of drama, intrigue, and difficulty.

If you play the game, many more things will make sense to you.
I don't really like to explain to the reader exactly what everything is, such as Luciferium or other drugs in the game such as Psychite Tea, or Go-Juice. I've always had the feeling of cringe when something is explained in a story that's based off of a game. I don't really know how to explain it, but you're just sitting there like 'yeah, okay, move on.'
But, even if you don't know much about certain things in the game, a simple quick search will bring you to your answer. Most of the Wiki is long, but it gives a short explanation on pretty much everything if you're looking for a quick, tight summary.
The game is difficult though. The story isn't ramped up to 11 at times just because it's for action. It's mainly because that's how the game itself functions. If you're playing on the hardest difficulty (or even some of the easier ones) thing's will go bad pretty often, and you have no control over it. It's a game that constantly tries to mess you up, and you have to do your best to get through it. It's a pretty rewarding game in terms of facing hardship, just how the souls games are held up so high because of that special feeling you get after killing a boss.
I'd recommend to play without mods first before diving into mods. The mods only add to the confusion if you don't know how to properly play yet, and it might cause some conflicting errors. But, if you're into getting over large, almost seemingly impossible odds, then you'll love the game. And, remember.
'Aint no one innocent on the Rim! So if you gotta' dive into some questionable activities, it's all in the name of survival.
