Specifically her testicles were left intact and she was also separated from her medications which suppress testosterone production. Given a lack of medication to suppress production and supplemental estrogen to further grind it into the ground, testosterone comes back with a vengeance. Given a lack of other options, the only remaining choice is invasive surgery to remove the cause of pain.
Being transgender is both implied and made explicitly clear back in chapter one when Liz slips up and calls her 'Sam' once, and then when the void dweller called her 'Samuel James Destefano' as she was cast into the abyss.
Being a 'futa' is further ruled out by the fact that the displacement left her with no genitals whatsoever, just gonads hidden behind a bony plate of armor.
