If she could grow and function just like a regular pony, then wouldn't she logically be able to regrow her, ah... skin and fur? If it wasn't somehow capable of regenerating, then it probably wouldn't have lasted close to two decades without anybody else noticing, especially with how much trouble she got into when she was younger.
If I were Spike, I'd probably do something as soon as possible to show that he's still into her. She doesn't sound very hopeful right now.

A cut, scratch, or burn that is small will heal after some time, but if it is too large, then a person will need skin graphs in order to recover. There's also the issue of not knowing how the synthetic epidermal layer works. Even if Sweetie Bot's repair abilities are more advance than natural biological beings, it's possible she would still need some of her epidermis or sub dermal layer to build on and grow in order to repair her exterior.

I am betting that Sweetie has some very advance nanotechnology in her. It's the only thing I can come up with that would explain her growth logically even as a stretch of the imagination.
