I have a few that might qualify. In Basking, Celestia is quite literally sick. In Control Freak and The Good You Might Do, Twilight is suffering from mental illness (though in the latter, she's not an alicorn yet). In Suffer in Silence, Celestia is going through something that I'd have a hard time deciding whether it's mental or physical illness. I have a couple chapters of my minific anthology that deal with a sick filly Twilight (obviously not an alicorn yet, and both are comedies), A Healthy Obsession and Last Rites.
