It's comic canon that Sweetie (and the rest of the CMC) volunteered at a hospital. It's also something I could see ponies doing: some hospitals have therapy animals especially for kids (usually dogs), which is a thing that a pony would be very good at. Not to mention that ponies are empathetic and ear-scritches are mutually beneficial.
Sweetie Belle is adorable, another point in her favor.
On the other hoof, she's Sweetie Belle, and disasters just seem to trail in her wake. Still, if she's not with her friends and crusading, things tend to go better.
