If I understand the lore right, Puppet was first via Possession, gave the original 6 life via Possession and the others were brought to life another way, assuming they aren't simply running on an AI for their programming. Circus Baby and Springtrap are also brought to life by possession, Baby by the killer's daughter and Springtrap by the Killer's soul. Nightmares either don't exist except in the fear of one's mind or are simply controlled to scare the son into staying away from Freddy's.
