I believe his name was always Red Eye.

I don't have a specific cutie mark but I always imagined it something completely unrelated to his actual end result based on his speech about them not mattering.

I like to think that he was actually born with two different colored eyes. One blue and one red, so they named him Red Eye. He would go on to replace his organic red eye with the cybernetic one we know him to have.
