I have multiple versions of chapter 16 that I've thought of and have partially written out.
In the first version Gerhardt tries to think of a way to protect Pixel since he knows he will occasionally have to leave town for business and won't always be able to take her with him. He's worried that something like the manticore attack will happen again and has lost all faith in the ponies ability to protect Pixel and Pixel's ability to protect herself and thus wants to find a way to ensure her safety beyond a shadow of a doubt even when he's gone. So he ultimately ends up very reluctantly having his factory build him another sapient golem, this one having the mind of an adult and all of his knowledge, to protect her whenever he couldn't.
In the second version of the chapter Silvanus get's frustrated by the current state of the war and ends up killing the other High Lords, except for Isabelle Sano due to the fact she is half elf and the fact she was the one he found the least irritating since she was just the High Lord of the Medical branch. So he decides to show her a bit of mercy and just sends her to Equestria. This turns out to be a good thing because despite the augmentations Gerhardt gave Twilight worst comes to worst and her condition starts to deteriorate. Isabelle catches Gerhardt up on everything that's been happening on Earth in his absence and vise versa. Gerhardt does not like what she tells him. In this version he would still be trying to think of a way to protect Pixel, he just takes a bit longer since he decides not to resort to making another sapient golem. Isabelle's arrival would provide that solution for him since she is a soldier just like him. Personally I prefer this version.
Just so you know, she would end up telling him that Silvanus sent her there, however he will have no reason to suspect Silvanus has anything tethered to him. He will just assume universes are orbiting around eachother like solar systems and galaxies and she just got lucky enough to be thrown out of the universe in a position where she ended up being able to latch herself on to the universe of MLP just like he had. This is because he will have no reason to suspect Silvanus had gotten rid of her in a way that was any different to how he got rid of Gerhardt in the prologue. Also neither the Greater Gods nor the Gods of MLP will care otherwise they would have gotten upset when Gerhardt arrived at the beginning of the story.
Though if you don't like either of these, I am more than willing to think of another version. I'm also open to any suggestions or criticism you guys may have.
