This is indeed quite odd. Maybe you should report this to FimFiction? There's no way that a wave of dislikes appeared out of nowhere. This seems like the work of some very dedicated troll.

What would they do?
I have not as yet read it - but if there was something very objectionable to most readers on chapter 4, that would just about do it.
If there was no great switch - 'my little dashie' turning into 'cupcakes' with no warnings - then something dodgy.

There was a simple story in that chapter. I've got a few likes in the day of releasing it. But four days after that I've got 15 dislikes in just one hour. I watched and each disliked appeared once in a few minutes. I would understand if some disliked it, but not all of them and not at the same time.
I was about to offer this criticism as a possible reason for the downvotes, but as the attack seemed fishy enough for the mods to step in and resolve it I am not sure if I should offer it.
Really, I don't think it's fair criticism, as it is quite unavoidable.
Some people get weird when ANY author brings in OC's. Which I think is lame in regards to those readers. Ever read Fallout: Equestria ? Soooo many OC's !!! Any pony we are familiar with gets a passing reference as they all died hundreds of years ago (this is not a spoiler - the fic mentions this upfront). And yet it is highly rated.
Myself ? I have no issue (ok maybe a slight issue as I am entering a story with no knowledge, but learning about new characters is part of the reading experience).
On a more personal note, in real life I have always had a problem with names and faces, which extends to books / tv and makes following any story with unfamiliar characters difficult. Perhaps a bunch of readers felt the same way and downvoted out of spite ? As I said, it's a lame way for a reader to go.
Stories with many OC's are different from non-OC orientated stories in that OC-heavy stories tend to be more on the "epic" side.
At least, that is my reckoning.
Authors don't want to tell a quick one-shot with familiar characters (and can't as new characters need development) so we get longer, more involved, more epic tales.
But... all this may be a moot point as some bugger with several alt accounts could be all this is.
I'm enjoying your story thus far, even though I keep forgetting who is the unicorn doctor and who has wings, etc...

The doctor is an earth pony and his student is a unicorn. But it's not that important for now.
And a little spoiler - there would be characters from the show at some point in the future. But for now it's an OC story.
