Thanks to Posted, I was able to convert the fic into an EPub really fast. I have taken steps to make it even nicer on e-readers, too. I've removed the redundant chapter names from each chapter. I've put page breaks in for the title page and foreword. I've also removed all the author's notes.
The icon, at least on my e-reader, is not showing the cover image in full, which is strange. I am not sure how to fix that.

I loaded the epub up in my reading app, Moon+ Reader, and the cover image displays properly for the shelf view. I also checked that the image was flagged properly with the editor I use (Sigil) and it is.
Must be gremlins.

I use a Kobo Glow. It's a mystery to me. The icon looked just fine when I used FimFic's epub exporter, so I have to imagine it's something to do with Calibre or FanFicFare. Also, it originally generated a generic cover, but I put in the cover image manually. Can't find anywhere in the epub or the metadata that would do it, so meh... it's not a big deal at all, but bugs me.

Ah, it isn't that the image isn't displaying, it is that it's displaying incorrectly. My guess for what is going wrong is that the software the device is using expects an image that is higher resolution than the one on Fimfiction (525x733). If you're already using the source image (1280x1920) I got nothing.
Actually, I do have one more idea. You have an actual cover chapter with the cover image. It is possible that your Kobo is pulling that page as the cover and it has extra white space.
And I think at this point we've collectively put more thought into this than it is worth.
