Some random MLP headcanons because why not!
The MLP World In General
-Ponies have last names like us, but some choose to go by both names or their first name only. Also, the parents either choose to give them the same last name as them or give them a new one
-Only Earthponies and Alicorns have hooves like real horses do, the rest have feet like in the show
-Alicorns are semi-immortal; they don't age normally so most side effects of age are not a problem, and they can be killed but not easily
-It's extremely common for flightless Pegasi and Unicorns who can't do magic to not have their disabilities taken seriously, since ponies ignore the differences in biology between the three races and go for the 'does that mean Earthponies and Unicorns/Pegasi are all disabled'. It's not uncommon for Unicorns to badly injure their horns in accidents and end up getting serious complications or even die because doctors assume that they're just Earthponies now.
-When ponies randomly improvise a song, it's actually a song that's existed for a long time, with the lyrics edited slightly for names and things. Randomly singing, or Dramatic Musical Numbers (DMNs) are a common occurence in Equestria, and it's customary to join in if you know the words (unless it seems like they're singing about how upset they are and want to be alone)
-There are some time travel spells that cause stable time loops and some that change the past and allow the caster to be immune to the changes. Twilight used the wrong one in It's About Time because she found it while panicking and didn't think to check.
-There are more males than we see, but the mares still outnumber the males. Due to this, LGBT ponies are more common than LGBT humans, but still rare and stigmatized.
-Equestria's moon has oxygen but not enough to keep normal ponies alive for a long time (think mount everest). Luna, however, isn't a normal pony. Nor is Nightmare Moon
-Alicorns can control whether or not they are immortal, and when they are the 'wavy hair' that they get when they channel huge magical power becomes permanent. In her first appearance, Luna was so guilty about Nightmare Moon she wanted to die but Celestia talked to her and changed her mind. Cadance is considering immortality but ultimately will decide she can't live without Shining Armour....only for Celestia to chuckle and ascend him. Twilight is immortal from the Element of Magic anyway but she might become immortal just for the cool hair.
-If a pony's colour scheme resembles a pride flag for any gender identity or sexual orientation, they are more likely to be that sexuality/gender for no apparent reason. The chances are even higher if there is no obvious genetic reason for them to have those colours. Rainbow Dash is straight, but her rainbow hair comes from her grandparents on her father's side, who were gay and conceived him magically.
-There is an odd phenomenon in the MLP world where if a filly is named Ruby, it is a near guarantee that she will be an emotional tomboy who develops a lesbian crush on a girly but distant unicorn called Sapphire whose special talent is seeing the future, and the first child they have will have Garnet in their name
Mane 6
-Twilight's full name is Twilight Armour Sparkle (she has two last names) but she prefers Twilight Sparkle because it's too much of a mouthful. Shining Armour called himself Shining Sparkle for a few years, then somepony pointed out the pun.
-Rarity's full name is Rarity Belle
-Fluttershy's full name is Fluttershy Breeze
-Applejack's full name is Applejack Bloom (the apple family has had several surnames over the years and call themselves the apples so they feel like a big family)
-Applejack's parents were killed in a freak magical accident when Apple Bloom was only a baby. She always thought her brother was lying when he told her about a purple Alicorn with a pink highlight in her mane and flashing lights. Twilight didn't use any time spells for months afterwards.
-The reason Sci-Twi/Human!Twilight wears glasses while Pony!Twilight doesn't is because Sci-Twi was exposed to computer screens as a child and eventually ended up damaging her eyes spending too much time staring at screens
-Twilight will not outlive her friends because the Elements of Harmony will make them all immortal
-The Mane 6 are all the same age, minus a few months. The exception is that Fluttershy repeated a year of flight camp and is thus a year older than them.
-Twilight is bisexual
-For most of her flight camp years, Rainbow Dash was bullied and called gay (she's straight, but being a tomboy with rainbow hair makes it easy for ponies to assume otherwise), and thus assumed that being gay was wrong. This continued until she came home one day as a teenager complaining about a school mate who keeps trying to hang out with her, but she doesn't like the filly because she's 'cross-eyed, clumsy, stupid and gay'. Her parents object to this and decide to introduce her to her gay grandparents without telling her that they're anything more than friends. After she's declared them 'totally epic' they tell her, and ever since then she's been the biggest ally, but she still hates being called a lesbian.
-If the ponies were humanized (fully humanized, not just Equestria-Girls-ified), Pinkie Pie would be black. Her normal hairstyle would be her natural hair, and her straight/'Pinkamena' hair would be cornrows. Don't ask how she does/undoes cornrows that fast, she's Pinkie Pie.
CMC
-Scootaloo's wings don't work because she's disabled, but she thinks she's just a late bloomer. Rainbow Dash and the CMC have many times persuaded her to go see a doctor and see if there's something wrong with her, but she ignores them. Secretly, it's because deep down she knows that saying you can't fly because you're only a filly ends with ponies thinking you're cute, and saying you can't fly because you're disabled ends with annoyed Earthponies and Unicorns (who don't understand that Pegasi evolved to sacrifice a lot for flight and without it they're helpless) saying that by that logic they're disabled.
-Apple Bloom stopped talking to Twist after the latter got her Cutie Mark. This led to Twist starting to hate Cutie Marks and will eventually lead to her wanting to start a cult that magically removes Cutie Marks, but Starlight Glimmer will set her straight.
-At some point in her life Sweetie Belle will decide to just go by her last name because she thinks the 'sweetie' part sounds too childish. It will be about ten minutes before a manticore kidnaps Rarity and offers to let her go and take Belle instead, and then refuses to let Belle eat at all unless she eats with him until she escapes. Rarity would have rescued her, but Trixie distracted everypony she tried to tell by singing about herself. Then she was promptly sued by Disney.
Nopony fights like Trixie, douses lights like Trixie, in a wrestling match nopony bites like Trixie!
-Sweetie Belle and Sweetie Drops are cousins
Side characters
-Starlight Glimmer has been afraid of time travel ever since she saw the wasteland she created
-Silver Spoon is a closeted lesbian, and she played along with Diamond Tiara's schemes because when they first met she had a crush on her and it took years for her to realize that Diamond was a bully. By the time she realized she didn't want to be with her, it was too late.
-Twist and Sweetie Drops are sisters, and Twist knew about the secret agent thing.
-One day, something will happen to truly get Maud Pie to show emotions and make her cry. That thing? Boulder's funeral.
Background ponies
-Derpy is a lesbian and is dating bisexual Amethyst Star. Dinky is the daughter of them due to some sort of sperm donor/magic/a wizard did it/it's a fictional universe flying-spaghetti-monster-dammit I can do what I like!
-Sweetie Drops and Lyra Heartstrings are dating. Sweetie Drops is lesbian and Lyra is Jack Harkness. After a while Sweetie Drops got tired of breaking up and getting back together and just decided that Lyra can be in whatever kind of nuts relationship with three dogs she wants and she doesn't care. They still have a duo catchphrase of sorts:
"Sweetie Drops and I are open to threesomes-"
"No we're not, you idiot."
They also have a heavy metal band called the Heartstrings. Sweetie Drops plays piano and Lyra plays the lyre, which sounds exactly like a guitar, because this is a fictional universe, flying-spaghetti-monster dammit!
-Doctor Whooves is the doctor
-Minuette is the pony version of Missy and Roseluck is the pony version of Rose Tyler. There are other pony versions to his companions we haven't seen, but the only punny name I can think of is Amethyst Pond. River would have the same name in the pony world though.
G3
Note: Many G3 characters have the same names as G4 ponies. Unless I state otherwise, I mean the G3 characters.
-Rainbow Dash and Minty are lesbians and RD has a secret crush on Fiesta Flair
-We're The Ladybugs has been stuck in everypony's head for several years after the one time it was played.
-Rainbow Dash has a British accent like in early G3 and honestly has no idea why.
-Both G3 Toola-Roola and Core 7 Toola-Roola are Irish and distantly Celtic
-Scootaloo and G4 Applejack are distantly related
-Scootaloo's Cutie Mark is because of the Butterfly Effect
-Scootaloo's infamously bad at doing hair among the group, and her signature low ponytail hairstyle was the result of an attempt to do a French braid
-Apple-Jack (my name for G3 Applejack) is actually good at singing (and a few instruments), shorter than average, and, in the words of Douglas Adams, is a hoopy frood who really knows where her towel is.
Other
-For the sake of convenience, even though in the toys it's always spelt Applejack, G1 Applejack spells her name AppleJack (capital J), G3 Applejack spells her name Apple-Jack, and for G4 AJ it's just Applejack. They also have different last names: AppleJack Klutz, Apple-Jack Picnic and Applejack Bloom
-If the mane 6 played FNaF (let's just assume this is EqG or there or computers or this is hypothetical anyway), Fluttershy would think the animatronics were cute, Pinkie would get up and (in-game) walk out because Pinkie doesn't follow the rules, RD would try not to admit that it's terrifying, AJ would actually not be scared and not get the big deal, Twilight would do a scientific experiment to find out how often to close the door for Foxy, and Rarity would just refuse to do it because it's 'too silly' (read: scary)
-If the mane 6 somehow came up with the idea to see who could continue a Genocide Run on Undertale for long enough, Fluttershy would utterly fail and end up going full Pacifist, Twi and Rarity would give up while fighting Toriel, Rainbow would give up and spare Papyrus, AJ would give in to frustration while fighting Sans, and Pinkie Pie would spare Sans. In the genocide battle. Without getting dunked on.
Interesting thoughts all around. In particular on why non-pegasi might think a disabled pegasus is somehow just 'bright down to normal.'
4505447 Thanks. I like to think that Pegasi have extremely fragile bones in order to be light enough to fly, among other things. Not to mention the fact that Pegasi who can't fly would be shunned by other Pegasi. It's not exclusive to Pegasi either; if Scootaloo was a Unicorn who couldn't do magic then Earthponies and Pegasi would insist that it's not a disability. It's a common thing in Equestria............hey, I should probably add this!

4535265 Well, there have been a few instances that imply all the others her age can fly (except Earthponies and Unicorns obviously), and although that could just be due to being a late bloomer, normally late bloomers would know at least one or two other Pegasi who can't fly, or only just started. Though it's mostly the fanon.

4537057 Well, in a society like Equestria, having disabled people would be rather stupid. Disabled ponies, on the other hoof.....well, according to the wikia, Stellar Eclipse (I think he's a voice actor's OC, but don't quote me on that) has a wheelchair, and the creator (who based him on himself) has spinal muscular atrophy type III, so there's almost definitely disabilities in Equestria. Not to mention wheelchairs definitely exist, and if they weren't generally considered to be for disabled people then they probably wouldn't need to invent crutches, especially considering how stupid crutches are on horses.

4537989 Well, it's not like other things don't exist because magic. We've seen racism, Cadance was pregnant (which implies sex), and a few other things. I'd be surprised if they didn't end up having a disability episode soon, probably one with the CMC learning to accept differences.
