Mane Seven (Spike included)
Twilight Sparkle: Twilight discovered her enjoyment of diapers and baby pony paraphernalia after her scare with the idea of returning to magical kindergarten. She discovered that being a baby pony was not about analytical thought, but about openness with one's emotions. She also enjoyed ageplay because it reminded her of forgotten memories before her magical training when Shining Armor would take care of her or act as a baby pony himself for her amusement.
Applejack: Applejack possibly plays the biggest role in this diaper pony continuity. She is uncomfortable with the idea adult foals and is even more shell-shocked when she discovers Fluttershy engaging Big Macintosh in ageplay.
She is willing to chastise Fluttershy for introducing her brother to such things, but when he stands by his relationship with Fluttershy, Applejack with a little help from Granny Smith is forced to ask how can she be honest about her distaste for adult foals and not lose her brother in the process.
These arguments also have a deep, painful effect on Applebloom, who is deeply ashamed of her own diaper usage. See Applebloom's profile for more information.
Rainbow Dash: She is introduced to diapers by Soarin. She mostly indulges his diaper fantasies, but occassionally wears when she is in a lazy mood. Her keeping Soarin's secret allows her to explore a new facet of loyalty, where she decides to stay loyal to Soarin despite him not fitting into the preconceived notion of who she had imagined he was. Pinkie Pie: She began to wears diapers as a prop in a comedy act she did when babysitting the Cake Twins. She subtlety discovers diapers offer her a more peaceful mode of enjoyment in contrast to parties. Later she takes the role of a playful baby filly when she engages in ageplay with Donut Joe. She also sometimes eats the magical sweets that Donut Joe makes for Soarin and becomes a fat, adult baby pony. See Donut Joe's and Soarin's profiles for more information.
Spike: Still being a baby dragon, he occasionally still has accidents. These mostly occurred during prolonged periods of stress. Secretly enjoys being treated in an infant-like manner, especially by Twilight and Rarity, but will deny it in a heartbeat. He also occasionally models infantile clothes from which Rarity can base her designs for actual baby pony clothes.
Rarity: She has recently discovered a love of making infantile clothing for baby ponies. She enjoys the challenge of trying to capture the personalities of clientele who are still so young within their clothing. As a result of this new endeavor, she had discovered that adult ponies appreciate infantile qualities in their clothing as well, as it allows to feel innocent and silly in a unique way. After discovering what adult foals are, she begins to create an adult foal clothing line. Her primary model during the creative design phase is Spike, and she uses this time together to both get work done and lavish him with affection. She will occasionally use diapers when she is particularly late on deadlines and does not have time for breaks, but even then she will find a way to make said choice fashionable and trend-setting.
Fluttershy: Fluttershy is attracted to Big Macintosh, and after she accidentally discovers his adult foal desires, she offers to act as his caretaker. This role empowers her, because she is normally shy, but she has confidence in her ability to take of him, which allows her to break out of her shell and become more open in general. Despite being the dominant one in her momma-baby role with Big Macintosh, there are certain moments when she becomes terrified of facing her own fears. At this point, Big Macintosh will diaper her and they will draw mutual comfort from each other. The events described here take place in the story, Big Baby Mac. Royalty
Princess Celestia: Celestia plays a motherly role to all of her subjects, though because of her chosen path in not producing a child of her own, she does occassionally express these urges by babying an adult foal. During the current era, Celestia has discovered one such pony among her royal guard. This pony is an OC Pegasus who is a member of Celestia's seemingly identical Pegasus royal guard. Celestia may also baby other adult ponies if the situation presents itself.
Princess Luna: Luna was the subject of Celestia's babying before she grew into her larger form. She still holds a slight resentment towards Celestia for the power dynamic that the babying represented. However, upon entering ponies' dreams after her exile, she understands Celestia's urges. She uses her control over dreams to help ponies overcome their fears (ex: Scootaloo) and allows ponies with young hearts to enjoy their fantasies. Occassionally diapers will appear on Luna if she is in a dream and she is subconsciously reminded of her own babying by Celestia. However, once she is aware of this, she quickly magically vanishes the diaper and insists that anyone who saw it was mistaken.
Princess Cadence: Cadence has begun to wish to start a family, but realizes that her obligation to the Crystal Empire is too great for her to do so. Shining Armor remembers when he used to play baby with Twilight as a colt and offers to act as a baby for her. Twilight's curiosity about the freedom of emotional expression acts a means for Cadence to baby her as well.
Prince Blueblood: Being the spoiled, pampered Prince that he is, Blueblood resisted potty training as foal. He eventually realized that wearing diapers as a colt made him look silly, and thus he finally put in the effort to potty train himself. Despite this, he remained in diapers at night because he was a prolific bedwetter and he didn't see the point of training himself to wake up at night because he figured he did not maintain appearances in bed (he didn't think ahead in that regard). He also secretly enjoys the comfort of diapers as well and he naturally slips into a infantile mode when diapered. It is possible that his haughty, narcisstic behavior is a coping mechanism for his feelings of inadequacy.
CMC, other fillies and colts
Sweetie Belle: Sweetie has played the role of a foal when she and Rarity played house when they were younger. She is the most open to the idea of wearing diapers and won't mind wearing them if asked for a reason she deems worthy, but overall she is more is interested in pursuing her adult side by finding her cutie mark.
Scootaloo: She was injured while performing a trick on her scooter. She was temporarily rendered incontinent and complained heavily about being bedridden at the hospital. Her rants against her recovery and forced diapering make Applebloom incredibly self-conscious of her own diaper use.
Applebloom: Unfortunately she has inherited the same weak bladder that Big Macintosh had as a colt. She used to wet the bed a lot more, but has been training herself to stay dry. Once she hears Diamond Tiara's jeers about bedwetters and hears Scootaloo's rants against her diapered tenure in the hospital, Applebloom forgets about being proud of the progress she has made and instead becomes fearful of anyone finding out about her own diaper usage. This leads Applebloom with the choice to either mock other ponies who wear diapers or to be honest with herself and to face her embarrassment.
Silver Spoon: She has only recently stopped bedwetting herself. She is willing to mock others so that she doesn't have to face her own embarrassment about her bedwetting.
Diamond Tiara: She begins the rumor that 'blank-flanks' are bedwetters to humiliate the Cutie Mark Crusaders. She doesn't realize that this strikes a chord with both Silver Spoon and Applebloom.
Snips: He doesn't wear diapers, but engages in plenty of other juvenile behavior, such as sleeping with a stuffed animal, so he joins Diamond Tiara's mocking so as not to be singled out. Snips' situation is meant to allude to Teddy's from My Little Pony Tales.
Snails: He doesn't wear diapers, but he also is childish like Snips and plays with stuffed animals. Unlike Snips, he's not ashamed of his behavior and thus views Diamond Tiara's mocking as wrong.
Pipsqueak: He is still a very young pony who sometimes needs daytime or nighttime protect through training pants. He didn't mind this originally, but felt bad after Diamond Tiara made fun of him for it.
Pumpkin Cake: Frequently uses her diapers because of her big appetite, but also constantly climbs out of them to mess with Pinkie Pie.
Pound Cake: Likes ditching his diapers to make trouble for Pinkie Pie, but he is not above using dirty diapers as "air bombs" on ponies he particularly dislikes.
Other ponies
Wonderbolts: They may choose to wear a diaper underneath their flight suit for long performances or flights as a means of testing their endurance and control.
Soarin: After Rainbow Dash attempts to find out Soarin's secret training regimen, she learns from him that his obsession with sweets (ex: apple pie) stems from his childhood, when he lost his parents. He grew up with his aunt, an excellent baker who mollycoddled Soarin and allowed him to stay in diapers. Soarin stayed like this for a long time until he saw the Wonderbolts perform and realized how pathetic he had become. He then began training and although it was difficult, he remained dedicated and was surprised to discover that as he lost more weight, he grew much faster. His wings had become surprisingly strong from carrying all of his fat, thus when he lost his weight he was remarkably fast and agile. His diaper had also added to his resistance training and had helped him learn maneuverability by teaching him to move with extra bulk. While this secret training was an embarrassing part of Soarin's past, it is also something very ingrained within him. When Rainbow decides to pursue a relationship with Soarin, he reveals that he enjoys returning back to his fat, adult foal form by eating magically enhanced sweets he buys from Donut Joe, who is one of the few other ponies who knows Soarin's secret. He enjoys being regressed, eating large amounts of food, and having Dash act in the caretaker role.
Trixie: One of the ways she classically humiliates ponies she challenges in her stage acts is using props to dress her victims up like babies.
Big Macintosh: Big Macintosh is adult foal who has inherited the weak bladder gene that has appeared in quite a few members of the Apple family. He was kept in diapers for a longer period than average for a colt and was in the process of potty training when he lost his parents. Their loss had a profound effect upon him, especially because of all the new responsibilities he gained in helping out on the farm because Granny was too frail to do it herself. Big Macintosh gave up a lot of childish behavior when he took over the farm work, but he has always been attracted to childish things, such as dolls like Smarty Pants and more secretly diapers and other baby pony paraphernalia. Big Macintosh has expressed a lot of self-hatred in this regard, and believes his. cutie mark shows that his destiny is to be a 'big' pony because it is a big apple. Once he and Fluttershy begin their relationship, she helps him accept his baby side, allowing him to finally become more open to other ponies and a role model to other diaper wearers.
Granny Smith: She is unwittingly the motivation behind Applejack's anti-Adult foal behavior. Applejack wants to protect Granny from undue stress, especially given the latter's infirmed state.
Granny remains quiet during Applejack's tirades against adult foals, but this is later revealed not to be because she believes Applejack is right, but more so her own guilt in feeling that she robbed Big Macintosh of his childhood. She seems to understand the true nature of his cutie mark and also prompts Applejack to honestly evaluate her stance on adult foals.
Donut Joe: Donut Joe is an old friend of Mr. Cake who comes to Ponyville to open another chain of his donut shops. He and Pinkie Pie develop a friendship, and he discovers her adult filly side. Her enjoys playing the role of her Daddy and introduces her to Soarin, for whom he had originally created his magical fattening sweets. This acts as the beginning of adult baby ponies beginning to actively meet up.
Cheerilee: She feels that the best way to teach young ponies about taking care of baby ponies is to use student or adult ponies as examples because they are generally less fussy and she tends to reward volunteers with extra credit or favors. She is also not above using student ponies in demonstrations as a punishment for 'immature' behavior. She is an expert diaper changer and expresses a genuine interest in adult baby ponies when she learns about the phenomenon, even experimenting with diaper usage as a means of exploring its mindset. She is particularly fascinated with Big Macintosh's infantalism.
Shining Armor: He has ended in up in diapers quite often for a pony that has never needed them for their actual reasons once he outgrew infancy. When Shining was still a colt, just after Twilight was born, he enjoyed looking after her, but when he noticed that she would get upset that she wasn't a 'big' pony like him, he would occassionally dress up as a foal to humor her and let her baby him to prove to her that she wasn't too far from being a 'big' pony herself and achieving her dreams. This kind act instilled within Twilight a sense of self-confidence that she values to this day.
When Princess Cadence sees some photos that Twilight's mother took of Twilight and Shining playing dressed as babies, she gets the idea to ask them both to play-act being babies so that she can prepare and experience the joys of motherhood between her royal duties, not unlike Celestia. Shining is first willing to play baby out his deep love for the happiness of his wife, but he is much more comfortable in the role of a 'daddy' figure when Twilight visits and allows Shining and Cadence the opportunity to play act as new parents together.
In addition to instances listed above, Shining has also ended up in diapers as a part of a Royal Guard hazing ritual. As captain of the guard he maintains this tradition because it subtly works as a team building exercise that get the team to look out for and care for each other on a deeper level.
Cheese Sandwich: He eventually returned to Ponyville to ask for Pinkie Pie's help in planning a very difficult party, which ended up being a baby shower. Everything thing he does is shot down by his difficult clients and he is worried about his reputation as a party planner. Pinkie Pie uses her insight to see that the clients are being difficult because they are worried about the responsibilities that come with having a baby. Together Pinkie Pie and Cheese throw a baby-themed party that assuages their clients fears that having a baby doesn't change who they fundamentally are.
Sounds interesting, so in this universe does Pinkie Pie having feelings for Donut Joe?
Thanks, I had originally intended for Pinkie Pie to develop feelings for Donut Joe. However, since the introduction of Cheese Sandwich, I've kind of shifted over to that camp a bit more. I could technically still have Pinkie develop a daddy-filly bond with Joe, but it probably would be more through the scope of a deep friendship rather than a romance.
I honestly like some of these head cannons. I've got my own head cannons for Scootaloo. I'd tell it but I don't know if you'd want to hear of it but if you do just feel free to message me! I also have a few for Silver Spoon and DT as well!

2890772 Well Scootaloo only wears diapers because of her older brother (My OC Rough Ryder) and both being orphans she just wants him to be happy for what ever happens. Ryder keeps diapers handy just in case he's alone and even for Scootaloo in case she misses her parents. (If you read 'A Pegasus? That's afraid of thunder?' It explains more about it.)
The Siver Spoon one is she wears diapers to bed after she drinks apple juice because it just goes right through her. Not to mention it makes her have nightmares triggering bed wetting.
Right now I'm in the process of writing one that includes Cadence and Shining Diaper head cannons. Cadence being a foal sitter might help with some trouble in bed~
Gonna at least try for an Applejack head cannon with night after night accidents in another fic i'm writing.
