Alright so it's been awhile since you guys have heard from me about Diane. Truth is I've had a lack of inspiration write since the last chapter.
university work has also got in the way. I've also had a few other ideas for mlp Fanfics, but I'm a bit reluctant to write them, as I'm afraid I might not be able to finish them, and also Diane is still on my table.

Also, are the equivalent of the MLP villains inside the Mental institute along with Diane?

Well for many people the time of the post is early in the morning when many are going to school or work which certainly limits the number of responses.
For me I guess it would be interesting to know whether Pinkie in her view of the world she though she knew is only like the show or was there additional things she has "done". Also if there was additional events it woul dbe intersting to see how these events may influence how she sees the real world. For instance if she had stronger feelings for a particular character (say Twilight, Dash, or BIg Mac) how would she react to them in this world since they now act and look differently than before.

Yeah probably wasn't the best time to post. Anyway to answer your question. Yes Pinkie Pie as a character is not exactly like the one in the show. The events of the show have happened, but there have been other occurrences in her life which would affect the way she sees the world, unlike Pinkie straight off the show. A big example is the forth wall jokes don't exist with my Pinkie Pie, but that's only just one example

Ah now there is an interesting question. With Pinkie Being in the high security wing, they are other potentially dangerous patients in there with her
so yes, your correct in assuming the villains are in the institute. I'm yet to reveal which ones they are but let's wait for the next chapter and see 
When's the next Diane chapter?

Annnnnnd second question. When I first came up with this idea, I did have in mind the idea that Pinkie actually had committed some form of murder, but at the same time I thought, that concept has been done to death. So I deiced just to be a schizophrenic thats prone to causing harm to others or her self, but as a character she hasn't killed anyone purposely or accidentally.
