If the text looks like this, it's narration.
If it looks like this, it's the character's thoughts.
"If it looks like this, they spoke it."
"If it looks like this, then the italicized words are under extreme emphasis."
"IF IT LOOKS LIKE THIS, THE CHARACTER IS YELLING AT THE TOP OF THEIR LUNGS."
"AND IF IT LOOKS LIKE THIS, IT'S EITHER MORE THAN ONE PONY YELLING AT ONCE, OR THE ROYAL CANTERLOT VOICE."
That's how I do it. It's how I've seen it done elsewhere. It is not, by any means, a rule, nor is it used everywhere. So, just in case I was accidentally using a set of conventions which you were unfamiliar with, here's the key.
