So, I was a little annoyed/bored/amused by the mock Army version of the form/wanting to photshop yesterday, and so after four hours trying to remember my old lessons, I came up with something I hope the users and mods of this site will enjoy:

It's available for download there, and if you want to use it, just convert it to a .pdf, fill it in, and post it on your blog! Though, if you'd like to tag/mention me as the idiot who created this, I'd appreciate it (business is slow...). It might help me get some story views, or might get me banned if the mods decide I'm being offensive, but hey, I hope it gives you a good chuckle. In case you need an example on how to use this form, I've put a filled-out form by myself below (and witnessed by myself-warning, contains very real butthurt, and a typo).
