Shao Kahn and Kitana have been alive longer than Mileena has (she is basically a child afterall). As dangerous as Mileena is, she can be a bit cocky, thus allowing Azula to clench the win due to her cold calculating nature. Shao Kahn is even more ruthless and has more forces at his command. Hell, if he wanted, he could simply devour Ozai's soul and end it there. And while Kitana can get cocky sometime, she might find a way to use Zuko's anger against him. Shao Kahn could then brainwash Azula and replace Mileena with her.
