I can only assume he'll need ether the Aeternus or a way to duplicate ammo, while it is possible to make a bullet using medieval tech it would be very difficult and time consuming to make just one. Considering that demons who can take magic magic explosives equal to rockets to the face and survive rome the land he would need lots of gear to even compete with a powerful wizard, though it would be interesting to see how some of his perks work in a new world.
Or you could have parts of the fallout world come through randomly appearing portals like at first it would start out as small things like ammo and stempack but the portals could stedaly get worse eventually leading to entire factions randomly appearing when there buildings gets sucked in eventually leading to large patches of land being uprooted and an apocalypse of two worlds colliding.
my advice on perks are
Lifegiver for the extra health and regen
Aquaboy/Aquagirl being able to breathe underwater could be useful when the entire world is not a desert.
Adamantium Skeleton for obvious reasons.
Solar Powered more regen.
Toughnessfor obvious reasons
Animal Friend and Wasteland Whisperer just to see if it would work on demons and spirits.
Blacksmith+Nuclear Physicist+Science! might make it so that plasma and or laser weapons could be powered by magic crystals.
I didn't search any of the other games for any perks so if you have any ideas tellme.
