I'm so very sorry for how long it's taken to get chapter 3 of Nightmare in Old Equestria going. Procrastination, distraction, and writer's block make for a devastating combination.
To try and provide something in the meanwhile, I thought I'd so a little something involving my personal Doctor Whooves headcanon.
Basically, the way I have it is that the Doctor in my fics has always been there, and has experienced most, if not all, the same adventures that our Doctor has in some form or another. That in mind, when it comes to companions, I have a specific rule: With a few exceptions (which will be noted) any Doctor companion from the "present day" will have an equivalent among the My Little Pony cast. Companions recruited from off earth or the far past or future will have OCs.
So without further ado, here's my Companion-to-Pony list:
FIRST DOCTOR
Ian Chesterton & Barbara Wright - Big McIntosh & Cheerilee
Susan Foreman - Perennial (OC)
Vicki - OC, but no name yet
Steven Taylor - Aerospace (OC)
Katarina - OC, but no name yet
Sara Kingdom - OC, but no name yet
Dodo Chaplet - If Derpy weren't pretty much the universally accepted Rose of Doctor Whooves-dom, a teen/filly Derpy would be perfect here. Otherwise nopony yet.
Ben Jackson & Polly - Lyra & Bon-Bon, mostly since I can't think of another pair of ponies likely to both join and leave together.
SECOND DOCTOR
Jamie McCrimmon - Persimmon (OC), possible ancestor to the Apple clan
Victoria Waterfield - Octavia (Exception), Victoria was recruited in the far past, but eventually opted to live in the present day
Zoe Heriot - Twilight Sparkle (Exception), Zoe was recruited in the far future. However she has some personality similarities to a pre-Ponyville Twilight (highly intelligent, initially antisocial (or at least not caring about being social)) and Zoe's fate as a companion is to have all but her first adventure with the Doctor wiped from her memory, allowing Twilight to be a companion without affecting FiM canon.
THIRD DOCTOR
The UNIT years would be some time before present day Equestria, so companions here are decided among those who are middle-aged or older in the present day.
Liz Shaw - Nopony decided yet. But I was just thinking of possibly Twilight Velvet (would make a nice choice since he's just coming off Twilight Sparkle as companion)
Jo Grant - Was thinking Pearl, Rarity's mother.
Sarah Jane Smith - Mayor Mare, based on this picture.
FOURTH DOCTOR
Harry Sullivan - Nopony decided just yet.
K-9 - Himself
Leela, Romana, Adric, and Nyssa are all from other worlds/far future, so they'll be OCs.
FIFTH DOCTOR
Tegan Jovanka - Not sure just yet. Though whoever is chosen would have to be describable as a "mouth on hooves." Almost want to say Rainbow Dash could fit here. Though unless there's a way to avoid interrupting FiM canon I'm avoiding Mane Cast members.
Vislor Turlough - OC, No name yet
Kamelion - Himself, since he's a robot anyway.
Peri Brown - Nopony in mind yet, though if I'm not mistaken she's from "present day" so she should have a cast equivalent.
SIXTH DOCTOR
Mel - Was thinking of Ms. Peachbottom/Chickadee, from Games Ponies Play. She seems to like traveling & certainly has the running strength to be a Doctor companion.
Evelyn Smythe - Granny Pie/Nana Pinkie, Pinkie Pie's grandmother. Inspired by a fic featuring her that gave me the idea of Pinkie's grandmother being a historian/collector of myth and folklore. Evelyn is a companion featured in the "Big Finish" audio stories, an elderly historian who pretty much invites herself into the TARDIS on the basis of she's a historian and the Doctor has a time machine. She also makes excellent chocolate.
SEVENTH DOCTOR
Ace - Princess Celestia (exception), the idea here is Celestia, in her rebellious teen years, didn't want her fate to be decided for her and so ran away from home. She transported/got transported to the Doctor Whooves equivalent of Iceworld and met the Doctor there. The Doctor took her on as a protege until she decided to return on her own to fulfill her destiny as Equestria's Princess.
EIGHTH DOCTOR
Grace Holloway - Rarity. Not a precise match, since Grace was a medical doctor. But I could possibly see it as a good match. Since Rarity doesn't actually leave with the Doctor she can encounter him without interrupting canon.
NINTH DOCTOR
Rose Tyler - Derpy. She's pretty much the universally accepted Rose, as the one most often paired with the Doctor as a love interest. Incidentally, I'm thinking "Time Turner" is in fact the Doctor-Donna clone.
TENTH DOCTOR
Mickey Smith - Nopony in mind yet. Though it'd have to be a stallion who hangs around Derpy (other than Time Turner)
Martha Jones - Nopony definite. But Nurse Redheart's the front-runner.
Donna Noble - The Great and Powerful Trixie (abrasive personality, develops into a better person with the Doctor, only to have her memories wiped just in time for "Magic Duel.") Also provides a nice contrast/foil to Twilight as Zoe.
Wilfred Mott - If not Trixie's actual grandfather, perhaps Pinkie's father.
ELEVENTH DOCTOR
Amy Pond - Scootaloo, in large part due to her status in fanon as an inconsistent orphan. Meets the Doctor as a filly, encounters him gain grown up, travels with him and gets involved with the 2nd Big Bang to get parents again. ("Scoot along, Loo!")
Rory Williams - Pipsqueak, due to wild mass guessing I saw on TV Tropes having him join the royal guard. Seemed fitting for the Last Centurion.
River Song - Herself, though perhaps naming her "River's Song" as more of a pony-style name.
Clara Oswald - Pinkie Pie because, let's face it, who else deserves the moniker of "The Impossible Girl." Though, in a twist, the Pinkie Pie that travels with the Doctor is not the Pinkie Pie that exists in Ponyville.
Feel free to share thought/suggestions/claims I'm totally off-base. I'll try and get the next chapter of Nightmare in Old Equestria out as soon as possible. I think I have a way to work around my block, just a matter of getting it done.

Although, I had a few recommendations for the rest of Eight's companions from Big Finish:
Charley Pollard: I'd have to go with either Daring Do or Applejack's mother, the former because she's an adventurer, and the latter because she's supposed to be dead (I know, bad reason for the latter).
C'rizz: C'rizz. (Because Changelings would be too obvious.)
Lucie Miller: Lightning Dust
Tamsin: OC
Molly O'Sullivan: OC
Mary Shelley: Twilight or Fluttershy
(So, does this look good? Remember, these are just my ideas, so it's perfectly okay for you to choose a different casting for them.)
