I think brain damage would be more fatal, and would get checked sooner than damages to the heart. Of course, water in the lungs isn't healthy at all.
She would only have to worry about brain damage if her brain were completely without oxygen for about 5 minutes (at least, that's the time for a human). Since she nearly drowned, but never actually stopped breathing, the most likely problem would be damage to her lungs from the water she inhaled. But that would heal in a few days as long as she gets some medical treatment, which she did.
But that's only if you want to get technical about it. This is a cartoon world, so I'm gonna say her greatest danger was probably from Pinkie somehow showing up under the ice with her and surprising her into accidentally inhaling more water .

Heart damage, from a sudden shock, could be; though it is rarely the case in a young, healthy person/pony. Had it been, say, Granny Smith in the water, then heart damage would be a valid concern. As every ER RN knows, checking more things is not a problem, checking too few can be fatal. I would be most concerned about pneumonia developing from the water she may have inhaled (assuming her larynx failed while she was shouting underwater.) [Fun fact most people who drown do not have water in their lungs at all. The larynx will snap shut to keep the water out. It is actually a big concern for rescuers as a hypothermic victim's larynx may not get the memo about being in the air again and remain shut. There have been a number of documented cases that a victim has been (semi-) conscious after being pulled out of the water but still "drowned" to death because the larynx did not relax to allow air to enter the lungs. (An emergency tracheotomy would have saved them if the rescuers knew what was happening.)]
