And wait, wasn't Pinkie's birthday "several weeks" later? What a great present - one of your best friends got seriously sick trying to make you a present. Oops.
1908510 212 weeks later... I agree with you there. If the Earth Pony hadn't come out, I reckon she would've gone around the lake to test the spell's effectiveness. However, should she have stood on a 'thin ice' zone, and put just a smidgen too much pressure whilst testing the thickness, the outcome would've been the same. The same, that is, minus having a nearby hut to enter and warm up in whilst a nurse is notified.
Or Pinkie Pie could've materialised out of a nearby bush and scared Twilight. Or one of Rainbow's tricks startles her. Or Fluttershy's inadvertent habit of sneaking up on somepony and talking quietly surprises her. Anything could've happened to make Twilight sink and freeze.
y'know, she could gone through the hole she fell through (unless it was solidifying just after, which would explain why he couldn't break through despite her back breaking it)
8845364 Sadly, a common cause of death from falling through ice is being unable to find the hole you fell through - people are known to panic from the cold and being underwater, and often lose orientation and sight of the hole they entered through as a result. It's a common hazard, and it's very difficult to pull them back through the same hole unless a) they are tethered to something above-water and can use the tether as a guide back or b) someone else jumps in with a tether and fetches them, which is as dangerous as you can imagine.
