I wonder if the twist will be that he is an inmate. It could be, but seems implausible considering the standards that have been set up. But the combination of strange actions by the ponies around him and the unusual circumstances he lives in and the ominous foreshadowing about his mental state seem to imply whatever is going on could add up to that. Or the twist will be what happened to Dr. Star combined with Mind Mend's dark seret, or relationship issues. I don't know.
