All obs in normal range.
Shivering ++ but normothermic.
RFL plaster cast in situ.
Patient rousable by voice, confusion and disorientation apparent. Patient reoriented.
Patient states that she remembers a "guy" telling her that he was going to "make me a mare" before being left alone in the forest. See hospital internal incident report #1357032 and interagency mandatory reporting tool report #F3479-432 for followup.
Nil pain or distress noted. Patient is resting comfortably ATOR.

Patient slept soundly overnight and was cooperative with care. Patient alert and pleasantly confused this morning, somewhat oriented but remains vague.
Patient aware of her disorientation and responded to queries by reading her ID bracelet. Green is very impressionable and when introduced to nursing staff claimed she was also a nurse.
IDC removed, tip sighted; awaiting void post removal.
Patient denies pain ATOR.

Patient appears anxious and requests assistance will all ADLs. Assisted patient with toileting (PUIT) and shower. Patient cries and states “I’m sorry for being so useless, I’m wasting all your time.”
Reassured patient and returned patient to bed.
Patient ate all breakfast items, requested assistance claiming she didn’t know how to use cutlery. Patient returned to sleep after breakfast.

Woke patient for interview with Lunar guards, Social Worker and myself in attendance.
Green reports that her name is Green (read from her ID bracelet) and answers all other questions about her life and family with “I can’t remember”, additionally stating “Maybe I wasn’t even alive then”.
When questioned about being touched inappropriately Green became distressed and denied emphatically that such a thing could ever happen. Patient was asked why she was crying and stated “You all have the wrong idea and I can’t fix it.”, “If I tell the truth you’ll all think I’m crazy.”, and “None of this is real anyway.”
Further questioning is to be postponed until Green’s mental state improves as per social worker.

Attended interview of Patient by Lunar guard. Interview was terminated by myself after it became apparent that Green could not answer questions in her current mental state and was becoming distressed by the subject matter.

At about 0330 Green was found to have wandered into another patient’s room and introduced herself as the patient’s nurse. Green appears to have imprinted on nurses as her herd and is imitating things she has heard nursing staff say and do in an attempt to integrate herself with the group.
Green startles easily to unexpected physical contact and struck her RFL against the wall when approached by RN to return her to bed. Patient denies pain/injury but appears teary. Patient states “I’m so tired”.
Returned patient to bed by levitation, patient remains asleep ATOR.

so she who was a he was a nurse and her mind defaltid to what she knew when confused. intresting. i wounder how it will be explaned if she begins to display knowlage of medical care that was unlikly to have been lernt in the current hospital or by one so young at all. There is also the question of if her magic will work the same as an avarage unicorn due to her past or might even not work at all

I'm a nurse so I use those abbreviations every day. I tried to tone them down a bit this time so it would be easier to understand.
I do have to look up acronyms at work sometimes, but that's generally only when a doctor is talking about an uncommon test or procedure.
Or when someone uses illegal non-standard abbreviations

That's why starting with the truth would be my policy in this situation. Others have taken the evidence they see, plus the story they've been fed, and come up with their own narrative to fit those facts-as-they-have-them that, while far from accurate, is far more mundane and likely than "I'm an alien dude from another universe who got zapped here and made into a mare by someone/somepony/something with eerie powers over reality, the multiverse, and everything!"
I'd rather they think me possibly-crazy than think me possibly-in-a-mental-state-it-would-be-rude-to-call-crazy-due-to-being-raped-or-something-worse for sure.
I do hope the next chapter comes soon.

I think that Green's mental state will decrease even more as the time goes on.
Think about it. It was an adult mind in a brain of an adult man. Now it is an adult mind in an unstable and not fully grown brain of a child. Plus hormones.
"Through the Looking Glass" dealt with this problem and showed how truly F-ed this situation can be.

I've worked as a trucker and in law enforcement. People using non-standard abbreviations all the time. My favourite is 'MT' as in empty or no load. Trucker logs have very little space to write things so the letters 'MT' will show up a lot. Trying to explain what 'MT' was to a dispatcher who didn't know what it was came very close to a 'Who is on First' bit.
Seriously though, too much jargon can be kind of a turn-off.

When I was a child I had my fractured arm reset under anaesthetic, and then I was given opiate painkillers. I can't remember anything that happened the next day but I'm told I was hallucinating and going around trying to convince people to help me chase an imaginary possum out of my room (a memory of something that had happened at home several months prior). I was pretty much back to normal the next day.
People react badly to anaesthesia sometimes, especially children and the elderly, and can sometimes take several days to recover, it doesn't mean they're permanently impaired.

Specifically, while there are certainly differences between "Real Life" and the show's universe, the other differences tend to have interesting effects on the resulting story.
Notable, Yes, Herds exist, though exactly which variant is unknown. Could be essentially a social group, a family, or the 3+ member marriage thing with a family. The imitation detail however, I am not entirely sure about. My first thought is that is a serious difference, but seeing as Human children imitate adults and other children, I don't think it actually is.

Given her previous conclusion of her status as a Clone, whether the body she remembers is still an old her or not, this makes sense. Not sure why she outright dismissed transformation, and of course, Ship of theseus and lack of continuity from sleep gets involved. Still, her seriously believing she didn't exist until she appeared in Equestria is.... Something.
Heh. All of those responses make sense from her perspective, and of course will just reinforce said wrong idea. This is not going to be resolved easily.

In this case the narrator is trying to make himself believe something. He doesn't want to tell ponies that he's from another world, he wants to tell ponies that he can't remember, so he has to convince himself that he has a past in this world that he can't remember, otherwise Fluttershy and Luna would be able to tell he was lying.

Both of those notes are from the same shift - the first was at 8:37am, they met with the lunar guards at about 11:25am and then the second note you quoted is written at 3:48pm the same day.

Yeah, I broke my leg when I was a child and had an interesting trip under the effect of a sedative. I was talking about catching a fish or something, I don't remember.
But I wasn't talking about the effect of a drug. More like biochemistry and psychology. Another body, another gender, another age it should feel like a hormonal bomb went off inside Green. Combine that with the severe levels of stress and you'll get one volatile mind.

Can i just say i love that you are a nurse and actually know what your doing with all of these notes and building a medical history. My mother is a nurse and i grew up with all of this so it is really drawing me in from a realism perspective to see nursing staff in Equestria to be treated and written properly.
Also that bit with Green falling into nursing routine was so adorable and hilarious.

Oh egads. I'm coming into this story late, obviously, but as a former EMT. . . geez you just had to plunge me back into medic-brain. . .
Don't worry too much about the jargon. Even though I have no experience in the medical field (other than a patient), I found it fun to read. And it isn't as if it contained absolutely critical information to the plot. Then it might have been a problem.

this is such a cool story. It's incredibly self-aware and dances around the pitfalls of a self-insert beautifully. I absolutely adore these medical note chapters. It puts Green's miraculous situation into such stark realistic terms. When green was hiding things from Luna and Fluttershy, her deceptions were carefully played out and --this is the part I like the most - they were not received the way she thought they'd be. Most self inserts are portrayed as intelligent. This is the first I've read that does this without making them right all the time. Green's judgements are sound and rational but so are the other ponies involved. I'm riveted. Can't wait to see how this story is handled.

Re-reading prev chapters and it's interesting that different jobs in the Healthcare field take similar notes. I work as a DSP (direct support personnel) — think workers who help people with intellectual/developmental disabilities with ADLs, chores, transportation, etc. Obviously won't claim to be a nurse, but we have to do case notes, direct support logs, behavioral reporting, abuse/neglect/exploitation forms if it's suspected, etc, and the way these are written reminds me of my own job. Ours typically are meant to be much more in depth, though, and we don't (typically) use all the abbreviations (depending on the Staff), and have some that are different (like VP for verbal promoting, CSS/CSP/DSP, etc). Currently trying to get into AMAP (Approved Medication Assistive Personnel) to be able to pass meds for when the LPNs and RNs are out, but running into scheduling conflict.
Little do they know, the little filly is probably more experienced than some of them.
