Doug's entire body was covered by the trenchcoat; they couldn't see that he was un-injured, and seeing the mare injured - who should have been the tougher of the two - made them think that something worse had happened to him.

Seems it was a more severe reversed gender scenario. I managed to follow along but I can definitely agree that the way this chapter pulled into the problem was abrupt.
I know Doug and Rarity just talked about it last chapter, but everyone was so out of the expected characters that their reaction felt jarring. I was expecting the pegasi to try chewing Doug out for possibly hurting Rarity. I mean, they practically just had the meeting about equal treatment/acceptance of other creatures and all of the sudden the fear they had has turned to overbearing worry for the unknown male?
Welp, I don't see a problem with this. Though it was kinda funny.

It wasn't so much pre-prepared notes or discussions I was thinking of (even I'd be suspicious of that kind of approach) so much as simply waiting until more was understood about the strange but uncannily similar world so that misunderstandings could be avoided in the first place by both recognizing and having the knowledge to explain the differences.
The biggest hurdles being how to explain humans being the only sapient species on our world, and how to explain the very significant differences between Equestrian ponies and terrestrial horses compared to the mostly superficial similarities. The ugly aspects of humanity I don't think I would actually worry about explaining since I actually find it impressive that the ponies have achieved a peaceful state for as long as they have compared to us. It's where technology overlaps where I'd want to be careful, but it's something I've approached in my own story drafts with some interesting results.
I feel clothes would be rather simple to explain in comparison though. Explaining the issues with modesty would probably be the trickiest part since there's a lot of nuance to it, but defaulting to explaining their function as protection would be easier. If nothing else equating clothes to the coat of hair ponies have would probably be the easiest way to go about it. Not only serving as a barrier to exposure to the elements (honestly, reading about him walking through a downpour with no clothes on has me wondering how he's going to get away with not getting sick), but also as a barrier protecting us from things that could hurt us. Something actually shown just now with Rarity's first injury only being superficial because of the cloth barrier, and not losing a chunk of her hoof afterward because of the thick boot.
On the royal guard, I regard a lot of what happens to them as a bad combination of the crises in the show being "out of their league" and Equestria being in a bad state of complacent peacetime. Big, powerful beings a regular pony couldn't hope to stand up to coupled with likely having no experienced combat veterans. In such situations I honestly think they would best serve as crowd control to get civilians out of the way and stand in harms way to protect them if necessary since they really couldn't do much more than that.
Though I'll admit I probably have a different approach to that as well. I've honestly never went along with the lopsided gender ratio since I've always figured it was more split between civilian and military coupled with every union in the show seeming to be monogamous like here on Earth. Businesses and government positions mostly being filled by mares with few exceptions while the military seemed to be mostly filled by stallions with the Wonderbolts being an exception. The gap later closing as the show went on as I think the actual ratio of unique on-screen characters by the end of the show was closer to 3:2 in favor of mares.
I did like Xenophillia, and I did get a chuckle out of that scene. Though I figured it was more of a rumor than actual fact. Honestly though, wished I'd gotten around to reading it sooner than I did (didn't want it influencing my own story drafts since it sounded similar to a sub plot idea I'd been working on).
In all honesty, I find this RGRE hilarious! Glad it was more visibly applied here. The surprise and swapped gender standards are one hell of a read!
Wow at first i thought that they were gonna accuse doug of hurting rarity. Im glad it was just that they were concerned he had been injured.....though they couldve handled it better lol
