I was thrown off by the mottled and bare skin, the truncated tail, the tentacles, and obvious spine and ribs showing through. It looks like my friend's mutant character.

I honestly don't care about Fallout Equestria any, so any resemblance is coincidental as I would not have seen the things you are referring to. I do apologize for the confusion. I will say that it is loosely based on this, though, with a few embellishments of my own. Sort of a mix between HP Lovecraft's ghouls and the Falmer of Skyrim. Pictured below is the ghoul, the main inspiration.
as I was reading the story thinking of it being related to the movie descent where the cave explorers were exploring and uncharted cave system and ran into ghoulish humans that started picking them off one by one
I should have known. I love Lovecraft, and Pickman's Model is short enough to read over and over.
I also enjoy the Falmer for the tragic backstory. These folks should be studied and see if they can be supplemented, like cats who just need taurine to live on non-meat diets.
