It wasn't so much a fetish to Rarity, as it was 'Rarity's darkest time'. She used ice cream to attempt to fill her depression, then sex to fill it, then both until she reached the point where she didn't feel 'depressingly empty' anymore. So yeah, the sequel would be after she got back into shape and the human keeping her from getting that depressed again.

No, the Princess would be a different story. Both stories could be considered 'sequels' to this. One is focused on Rarity and the human after the event of this. The other with the Princess being depressed and a human cheering her up in the same way as this, but Cake instead of Ice cream. Lots of cake.
