Unlike the unnamed protagonist from Four In The Morning, this story didn't come from a dream or just flow from start to finish. The genesis of this story was a crackfic idea:
Equestria Girls Flim & Flam offer patients in need of organ transplants 'guaranteed perfect donor'... for a price. They of course raid Equestria for these organs, bringing the ponies through the portal and uh, robbing them of the needed organ(s).
That idea is up for grabs because I'm never going to write it. It's not my kind of story - I probably wouldn't even read it, let alone write it. Maybe it's already been done? I haven't seen it, but I haven't gone looking for something like it, either.
That got me to thinking about 'what if?' scenarios with a human (or pony) in need of an organ transplant. The perfect match is on the other side of the portal, waiting for them. So then I greenlighted the idea and started writing the story.

I really wanted cover art to show Sunset Shimmer standing next to a bedridden, comatose Cozy Glow. But Cozy never appeared in Equestria Girls.

I'm not an artist, and I stretched the absolute limits of my capabilities editing the image accordingly. It's passable, as long as no one scrutinizes it.
...Cue everyone going back to scrutinize it.

Yes, I know it's yet another Cozy Glow story. That said, it's only the second time I've written anything involving Sunset Shimmer, and the first time I've ever used the Equestria Girls tag.
You'd think I'd make it a point to write in the Equestria Girls universe more often since it gives me an excuse to write stories involving cars. Maybe one of these days I will... but I wouldn't bet on it. Especially considering that I completely blew the opportunity to write about cars in this story.

Hey, some of us like Cozy Glow. 
In all seriousness, I find the character fascinating. With no backstory to explain this perfectly innocent little angel*, it's up to the rest of us to create our own headcanons. As can be seen in my back catalog, I have many (often competing) theories on how she ended up this way.
*Only in her own mind... or on Opposite Day.

Tirek and Chrysalis groaned as Cozy twirled around the living room, singing and dancing. Sunset chuckled at the girl’s enthusiasm. She’s going to need it.
Silly Sunset, Cozy isn't being enthusiastic, she's being intentionally annoying. Chrysalis and Tirek are her favorite kind of audience: a captive one.

Don't worry though, their editor loathes Cozy Glow to even it out.

Look on the bright side - at least I don't ask you to edit the Cozy Glow stories.
What I always hated about Cozy Glow was that we were never given any motivation for her, at all. She was just "Diamond Tiara, but more evil." That's it. A CHILD antagonist, it just felt WRONG to never get ANY backstory about her, AT ALL.

She's a miniature pony pretending to be a child? Never said on screen, never implied, and NOT EARNED!
As long as w're mindlessly rambling, I felt Celly and Lulu dumping all their responsibilities on poor Twilight and not even giving her a hundred years to peacefully transition power was horrible writing. The show REALLY went down hill in terms of writing in the final seasons.

While she's a bit of an unreliable narrator, she does specifically say 'I'm just a kid' to "Grogar" when he questions the gathered villains' previous failures. So canon has more-or-less rendered the theory that she's a Babydoll-style adult that looks like a child a moot point.
I think they should've ended it after season 7. It still would've been after the show ceased being relevant, but at least it could've ended with some dignity. There are times I think it would've been better off ending after 65 episodes, as originally intended. Leave the audience wanting more (perhaps wrap things up with a theatrical movie featuring Tirek as the villain, but leveling him up in badass/creepiness like G1 Tirac since they wouldn't have to conform to the Y rating).

Episode 100 was a niece piece of work. And I think Kevin was proof enough that changelings had free will and could exist peacefully with ponies... WITHOUT making them pastel color swirls.
