Fitting my characters into the universes of other authors is tricky. My "Safe Landings" series had plenty of room to add fanon without affecting the main story. It's not so easy to do that with most other stories unless mine are set after the conclusion of their stories. One of the few possibilities that I like is the Royal Beanverse and I'll have a think about that one.
The safest direction though is to come up with something unrelated, even if it turns out to resemble something that another author has done before. With tens of thousands of stories on this site, it's impossible to know them all. Not even word searches are always helpful. I'm guessing that there could be universes where humans and ponies have always shared the world, whether it be in harmony or conflict.
One idea that I had was that all pony births are twins - a colt and a filly. Their lives are intertwined with each other, psychologically and socially, and the death of one could be devastating to the other. In this universe, Celestia would be a stallion while her twin sister Luna remains a mare. But what other pairings do you think there would be? What differences would that make to history? For the sake of argument, we will ignore reasonably small age differences. Big Macintosh and Applejack would be twins. Shining Armor and Twilight Sparkle would be too. Let me know your pairings. And in the case of same-sex characters, which of them would become the other sex.
EDIT: If the parents of the characters is unknown, then the foals can be of mixed race.
I'll keep a running list of the fraternal twins with the male sibling named first. These might be your suggestions or my own preferences. Not all are fixed in stone yet.

You've reversed genders of everyone. How about Luna as the older sister or Twilight being older than Shining Armour

Sunset and Cadence. We know that Empress Amore was a unicorn, so there is that. It would also help the previously setup story so that Sunset's jealousy stems from Cadence achieving ascension before she does, or Sunny just being unable to(hence her running away). Either could be male or female.

I like this idea. Sunset would have to be the brother though because I just can't see Cadance being male in this situation.
Another, much more convoluted idea: Trixie and Twilight, with Shining Armour being a cousin and twin with someone else... Like Fancy Pants

I get what you mean. The fraternal twin that was born first is considered "eldest" and this carries a great deal of weight, culturally. In each applicable pairing, the youngest in the canon universe becomes the eldest in the alternative universe.
I can see bulk biceps and thunderlane being twins bulk being the male and thunder being the female, along with Rumble and appleboom or scootaloo if you wanna keep the pagasi together, and Sweetie bell and Diamond tiara could be funny.
other pairings include(in no particular set) octavia vynal, Cheerilee, berry punch, Carrot top, and Trixie,
canonicly pinkie is already marble pie's twin sister, remember her comment about her being the older sister 'by a few seconds'
also sunset would be better as sunburst's twin thats a common fannon anyway
and while i like the idea of trixie and starlight as sisters, that precludes there rather popular romantic ship.
also your gonna wana rethink dinky, the fandom bond between her and derpy (aka ditzy doo) is too strong, unless you want to make ditzy rarity and sweeties mom? that would be....interesting

I'm really stuck for a pairing for Sweetie Belle though if Dinky is not a good choice.

if you wanted a bit of irony you could have chrysalis be cadance's twin, changelings gender isnt really set in stone after all it wouldnt be a stretch to think that chrsy prefers a female form and one of hte reasons 'she' became a changeling (somehow) was to be able to be the gender she prefered, could also be a reason for animosity between her and cadance as she rather illogicly blamed cady for her being born the wrong gender
lets face it in most continuties chrysy isnt the most mentaly stable so its not like that is a stretch
How about not necessarily Luna as Nightmare moon winning or reversing that so Celestia is the one that is corrupted. How about a universe where Night ponies like Thestrals are the majority and the other tribes are in the minority
Cheese Sandwich and Maud Pie, maybe? Pinkie's already a twin with Marble, so that blows that idea out of the water.
A bigger set of opposites I can't think of.

Well that's a fantastic way to get my curiosity up to a boil. What could the great Goldfur possibly have in mind?
Blaze and Spitfire? Spitfire's potty-mouthed sister ends up as her brother...
Bright Mac and Kinky Twist? Kinky was one of Bright and Pear Butter's biggest friends in school (non-canon, but who says we have to use all canon characters?), so if she was Bright's twin, then Bright could still marry Pear Butter and she'd be Mac and Applejack's Auntie.
And you have the very rare set of quadruplets.
