Because it isn't real friendship. It's what happens when your crush doesn't know what the heck to do with you so he or she offers a sort of pseudo friendship that isn't really friendship at all but more or less is a one sided friendship with the crush being friends with you. You just put up with and go along with this fake relationship, putting on a smile because being in this weirdo bizzaro land of being able to be close to them without being intimate is the best you can do.
Long story short, the reason it hurts is because it's fake as shit and only the crush believes it's friendship at all when really it's just a torturous hell where you're forced to hang out with a person you can never have on a day to day basis and you could never tell them that you don't want to be friends because not hanging out with them is more like a slow painful death and you would always rather than an eternal torture chamber.
