So....
....the next patient was going to be Luna. And Pinkie was going to deal with her depression and sense of dishonor. And... I really had a hard time trying to figure out how to approach it and I kept putting it off and writing other things and...
Well, then Do Princesses Dream happened. Which basically scuttled my plans.
SO. If I were to reinvigorate Pinkie's side job, I'd need a new patient. And I've always relied on reader input to decide that. That's what this blog post is for: Voting for Pinkie's next patient. You lot have got till the end of the month to put something in the comments below. Toodles!
What sort of psychological issues would Scorpan have between his scene of morals that he developed and his original desire for conquest? And would those issues last for a long time?
Maybe Coco Pommel needs help getting through her shyness?
Bulk Biceps has an irrational fear of butterflies. Would Pinkie have a reason to look at that?

Edit 2: Twilight herself. After the events of The Lost Treasure of Griffonstone, Pinkie may try to help Twilight with her need to be preforming official duties. Impostor syndrome issues that Twilight is doing official duties to try and ignore/prove her worth, perhaps?
THIS IS STILL HAPPENING?!?!? I'M SO HAPPY! This was one of the stories that first introduced you to me.
Anyway, Luna's minute long confession didn't solve all her lingering baggage and issues. I'm sure she still has plenty to explore.
But if not her, how about daring do and hows she restly is compelled to seek adventure and artifacts .
Hmm -- We have Rainbow Dash this winter without Tank. Diamond Tiara or Silver Spoon could be interesting. Queen Chrysalis could be cute. Spike has to have a bunch going on in there. Fluttershy could always interpret for one of her animal friends. Gummy could be interesting given his recently shown philosophical bent.
One thing that was never addressed in the show was how Discord's betrayal affected his relationship with the mane six. You might try having Pinkie delve into Discord's feelings about the main six, especially after the events of the Smooze episode.
Also, since Bon Bon was a secret agent, she might have some form of PTSD that Pinkie could help her with.
The Doctor mentioned a traumatic event in his childhood leading to his love for science.
And you might consider explaining why Vinyl Scratch isn't speaking. At all.
3330326 I second the idea of Coco. Only because she's adorable.
I suppose it all depends on when you plan on having it take place. The last time you updated this fic, Twilight wasn't even an alicorn yet. It'd be a pretty damn significant timeskip to place new chapters not just after Twilight's ascension, but so far after her ascension that she's beaten Tirek and got her pretty princess treecastle and done all the things we've seen so far in Season 5. Especially since, if I remember correctly, the entire premise focused on Pinkie's rather immediate monetary issues.
I have to read it again to reconfirm that. It's been a while.
That said, me personally? I'd refrain from such a massive timeskip. Maybe build up to the later seasons over the course of the story but I feel such a significant leap might be jarring. Making a suggestion with that in mind, I'd go with Princess Cadance and/or Shining Armor dealing with issues after the events of the Changeling Invasion. And maybe other things. Depending on your interpretation of alicorns, Cadance might have a lot on her mind.
If you're going to go ahead with the timeskip, then I'd say any of 3330447's suggestions would be reasonable.
Whatever you choose to do, I'm glad to see you getting back into this. This story was the whole reason I followed you in the first place :)
I vote Hugh Jelly for the win. Because as far as I know, no one has given him decent characterization, and you'd forge ahead in unknown territories
I wouldn't mind if it stayed in the early seasons.
Maybe Blueblood has a reason for his behavior, same as Trixie.
Might not be a good reason, but still.

DISCORD, not only the betrayal and his obvious abadonment/attachment issues, but the fact he is the spirit of CHAOS trying to find a place in a world of extreme order (I'm sure he wishes the entire world was the everfree). He just can't stop being himself, he needs it like others need food
I still think you could go with Luna. Just post-Tantabus. Like, there has to be more than what was fit in the three-minute epilogue of the episode. It could be dealing with the aftermath, or at the very least, a check-up giving Luna a clean bill of mental health.
Please. I want to see more Pinkie-Luna interaction.
