You guys know what the 5score group is about. For those who don't, its about people who were apparently ponies, but cursed to be humans for 25 years, before suddenly going back to pony again.
And then I started daydreaming about people who have bits of foreign material in them. This is not food I'm talking about ... actually, that would also be a problem. What if his/her "pony form" was deathly allergic to mangoes, and had a whole bunch of it just sitting in your stomach?
What if you had a pacemaker installed, or a heart transplant, whether artificial or flesh?
Metal pins from a broken bone?
Prosthesis?
In the middle or just after of a long, invasive surgery, with a whole bunch of foreign blood and stitches possibly in you?
That sounds to me like quite a few hours of agony as bits of things start getting shoved around where they shouldn't be.
I should stop.
Mentioned this on Skype already, but a bone-marrow transplant going septic and just barely held in line with the pony's own magic fighting back could be a really effective ticking clock for a plot.
Can they find a pony relative in time, and will that pony even agree to something like that to what to them will feel like a total stranger?
And then of course, wouldn't a pony giving up a bit of themselves expect their 'new' family to come with them and fight for Equestria? No matter what the protagonist —perhaps even his/her human relatives, might say about that?
Think that could be rather compelling.

Um, yeah, you see, the problem with me trying to write that is
1) I'm a terrible writer, and doing that kind of character dynamic is going to suck unless I get a LOT of outside help
2) The only scenarios I can think up that would amuse me is
--a) Every single bit of bad luck, aka driving alone on the way to Las Vegas, crashing from vision seizure, getting carted off to a hospital after a long drive, going through a minor surgery in order to put restraining pins on a broken bone and some blood transfusion and then a few hours later dying to everything clotting up. Maybe also while screaming at the pins getting jammed somewhere.
--b) The relative/friend that gets roped into fighting for Equestria is Adam Jenson. Because something something magic and Sariff being an inner dork.
Oh, and going septic in HiEC would also be amusing.
There is a line I have clearly passed, but let's keep going and see what happens.

You never know if you don't try, and you never grow if you don't keep pushing yourself. Trite old words, but true.
Still, if it's not the type of story you have any interest in writing that's a whole other matter.

Not that I'm one to judge with my own Five Score and Dresden Files crossover, but I'd read the hell out of that one.
All jokes aside, I think that one actually could be one of those 'bacon and chocolate' type deals, where it sounds utterly mad but the resulting mix is actually quite good.
Heck, people forget that bit with Adam's backstory quite often thanks to all the cyber badassitude, but he's a security expert. And damn near a one-man top-dog in that field, at that.
"Ma'am, are you aware of something called the 'long-lost friend con,' and that I'm not an idiot? You want a cybernetic mercenary, go hire Belltower."
"But it's your destiny!"

Or him being so "not-human" anymore (heh, transhumanism joke) that the magic field doesn't recognize him, and therefore completely ignores him, leaving him still human.
I also haven't actually watched or played DX HR, so I'm a bit lost personality wise.

Well, if the same 'curse' can warp flesh, bone and even DNA without as much as discomfort over a period of days, I don't think it's that much more taxing for people's imaginations that the same magic can warp and twist titanium, circuitry and carbon the same way.
Could even be a plot-hook. Anybody with cybernetics before ponification now being worth millions if not billions, due to their cybernetics having warped with them. Turning those augments into strange amalgamations of magic and machinery; the type of cybernetics ponies would have made if they had the same tech-level as DE: HR humanity.
Granted, since one of Adam's things is that he's special genetically speaking and can take far more cybernetics than the norm, that would probably result in this:
