Or rather frustratingly, it hasn't happened yet.
This week, all my gaming time has been dedicated to getting the "That Just Happened" achievement in Halo: Anniversary. To do so, you have to fight through the four floors of The Library on Heroic without dying. For the unfamiliar, The Library is Halo: Combat Evolved's seventh, most difficult, and worst-designed mission.
Here have been my attempts so far:
1. 1st floor, surrounded
2. 1st floor, surrounded
(at this point, I slowed down considerably)
3. 2nd floor, grenade
4. 1st floor, grenade
(at this point, I stopped being so grenade-crazy)
5. 3rd floor, point-blank rocket launcher
(This was just bullcrap. It turned the corner and there it was, point-blank. I died so fast, hitting the jump button actually skipped the death animation and reset the checkpoint.)
6. 2nd floor, surrounded
7. 3rd floor, surrounded
8. 3rd floor, flood carrier explosion
(Brady was watching this one via Skype. A grenade sent a carrier airborne and landed directly where I was backpedaling.)
9. Co-op, 1st floor, Brady surrounded
(We blame this on a faulty mic on my end, unable to communicate effectively.)
10. 4th floor, surrounded
Now that Brady has gone and done it by himself today, I'm gonna make a few more attempts, NOT following Marque's advice. Last I checked, doing it over and over again on Heroic prepares you much better than doing it over and over again on Normal. Who would've thought, eh?
Still, I haven't tried for an achievement this many times since Mile High Club in Call of Duty 4.

This one and to complete Slender The Arrival on Hardcore difficulty gave me the most difficulty. Luckily, The HMCC in November has different achievements.
A total of 402 achievements. And I'm going to get. Every. Single. One.
I remember The Library.
I played it for days because it was so boring.
I remember getting so confused the first time I played that level I accidentally ended up backtracking through one of the floors about halfway through it... without realizing I was backtracking.
