Some are forgotten, some are forbidden to speak of... Some are even buried into history purposefully. However, one journal with an unknown writer was found at Twilight's library. Here, some ponies have said, was where her death report was found after being lost.

Some ponies have wondered if mare mentioned above has tried to commit suicide before, but there is no evidence to back this up, so this option was discarded. Ponies believe that Twilight was poisoned, but I doubt that. The doctors performed an autopsy to figure out how she died, but nothing showed up. In this journal contains odd handwriting, but no pony knows who (or what) the handwriting belongs to. Another journal was found that was written by Twilight Sparkle herself, but some pages are missing, it looks like. The writer of the first journal sounds as if he or she was never in Equestria before, but knows how things go down. It doesn't seem natural.
