Good question! Obviously Twilight plans to help, but Twilight knew that Starlight and Sunset had a lot in common, so she took it upon herself to assign Starlight a friendship lesson. Twilight thought it would be good for Sunset to have a friend in Equestria she could really relate to, both being reformed villains and all, and struggling with their guilt and to understand friendship.
Very similar to Starlight and Trixie, but the difference being that Twilight is encouraging from the start.

Yes, it is a very short chapter. I think this is meant to be a sort of 'interlude' or 'meanwhile...' type-deal. I really wanted to express Twilight's remorse and her insecurities at some point before it potentially all gets resolved, so I thought this would be a good little transitional chapter before Starlight and Sunset return to Equestria.

Oh, in case anyone hasn't made the connection, Doctor Horse's pony counterpart appears in the Season 2 episode "Read It And Weep", the one that treats Rainbow Dash when she gets a fractured wing.
Here, the human Doctor "Ryder" Horse is slightly more pessimistic and grouchy. For comedic purposes, mostly (think the real Bernard from Megamind). It'll be hilarious making him struggle as a pony, lol.
This is probably the most characterization he's been given in any fanfic ever, but I could be wrong!
