I was skeptacle at first, but gave it a try anyways. It's really good, in my opinion. Don't fully judge a story because of a single trope you dislike, there's more to the story than just that!
I just realized two things.
The author says this chapter is canon, meaning Luna now knows that 1) Green was for sure an adult (though she likely already suspected this due to the early chapters where it's said "I haven't feared my dreams since I was twelve"), and 2) Green is revealed to have originally been male (references to "boy" and "son" as terms used for her/him). Even if ponies don't use terms like boy/man, "son" is a word they'd recognize.

They were saying they didn't like the "human male turned into pony female" thing, said it was overdone.
First comment I believe was them saying "It better not be this. Its overdone, why can't it be the other way around, yadda yadda." The second comment was them quoting a part of the story's first chapter where Greene found out that he was now a she with a ton of "..............." followed by the words, "Yeah, I'm out."
Not really an issue I think you can fix, just something of their personal preference, and they took a poor route of venting it.

Bluntness is saying what you mean.
If you mean "Disappointed, I've seen this too many times before. Going to drop this one."
Saying "this had better not be X" has an implied judgement that I should not have written the story this way, which is what my readers seem to be responding to.
It's the difference between bluntness and entitlement.
I like how the story is going so far. I did take a break part way through so there are parts I don't remember anymore. I am still enjoying the story. It is very good.

It depends a lot on where you work. Most of the actual information where I work (at least in terms of nurse observations) is all contained within other forms with checkboxes and dropdown menus, obs also have their own seperate chart, as do medications, so the progress notes end up being quite brief. At another facility I worked at the notes were more similar to what Dr Red Cross wrote in the first supplemental chapter with subheadings within the notes.
