It's more like removing pain receptors mean you don't know when you're getting cut, or burned, or have a broken bone, or any injury really.

There's a thingy (Congenital analgesia) where someone is born without the ability to feel physical pain (more or less). Looking at that in humans, you can kinda tell what would happen to Twilight when she yanked her pain receptors out.

Apparently in some rare cases, you also don't have the ability to sweat. No clue how much that would affect a pony/horse though, I'm not sure they can even sweat in the first place.
